Bulletin Brief

During routine NDT inspections on an offshore facility, an IRATA L1 technician raised a HOC with GR
Management and the client, highlighting that the emergency eye wash station near them was not working
properly at site. INVESTIGATION NOTES: During investigation by the TA, it was found that the technicians
were using the water from eye wash station for their couplant gel. While the site L3 had prior permission from
the control room to use this water source, this activity is strongly discouraged, as most facilities will run alarms
to the control room when these devices are in use. It is advised to never use the deluge stations unless in the
case of emergency.

Immediate and Associated Root Cause Analysis

1. Misusing of emergency eye wash station.

2. Negligence by the technicians while performing the given tasks.

Corrective Actions

1. Mail sent by country TA and Group TA to the team members, regarding the proper usage of emergency eye
wash station.

Preventive Actions

1. TA discussed with all technicians on site and established their clear understanding of emergency eye wash
station. It is advised to never use the deluge stations unless in the case of emergency.
2. Company Safety Bulletin to be circulated to all company offices / sites on the topic.
