During routine installation of walkable safety net works at a Ras Abu Aboud Stadium, an IRATA L2 technician,
was threading ratchets straps into the netting at ~5m height from the bleachers. The technician sat on the net
and detached both personal lanyards (one with carabiner, one with enforcer), from the available temporary HLL.
The client and GR site safety officers noticed that technician then attached both lanyards to the nearest ratchet
strap installed on the nets. GR Safety immediately informed to IRATA L3. The IRATA L3 advised the tech and
immediately stopped works and a written toolbox talk was conducted on the site.
INVESTIGATION NOTES: The technician stated that he was not aware of his actions or of being absent
minded, however the deliberate action points to a possible abuse of, and fault with, the existing access system.
Further investigation established that the multiple HLL’s installed across the net, did not provide for easy and
access straight down the length of the nets when threading the ratchet straps, and instead meant excessive
travel back and forward on other HLL’s. It is then suspected that the techs developed the habit of detaching and
then re-attaching from the HLL’s, moving down the net, from one HLL to the next, to speed up the process of
access and install.
Corrective Actions
1. Work was immediately stopped, a verbal toolbox talk conducted with the team regarding the requirement to
attach 2 points.
2. Rigging and access re-evaluated for the site.
