LPD Udhaliyah Area Construction Safety Team conducted a regular field safety inspection along with contractor safety staff at RO & STP
Area - TCF3. We would like to bring to your attention on some deficiencies that we noticed during the inspection. These observations
were notified to SAPMT OE Compliance Coordinator Mr. Kehinde Adebayo. We appreciate your immediate action to address them in a
timely manner.

Findings & Recommendations

Finding 1
At Reverse Osmosis Tank Area
No water & disposable paper cups or separate cups available at water station.
Recommendation 1

Site Management, Supervisors and Foremen shall:
Provide drinking water stations (e.g., coolers with chilled or ice water) for workers and remind them to drink plenty of water even if not
thirsty. (Reference: CSM I-13; Section 13.2.1-G)
Provide personal water bottles or insulated containers (2 liter capacity) to workers as needed (e.g., during high heat stress danger
categories). (Reference: CSM I-13; Section 13.2.1-I)

Finding 2
At Reverse Osmosis Tank Area
No permit receiver was available at site to supervise the work activities. (Unavailability of permit receiver at site is a repeated infraction)
Recommendation 1

Receivers are to remain on the job site at all times. However, if a receiver must leave the immediate work area, he shall give the permit to
another SA-certified permit receiver.

Finding 3
At Reverse Osmosis Tank Area
Safety infractions in relation to PPEs:
1. One worker was cutting a sheet of rubber using a cutter without using hand protection.
2. One worker was cutting a piece of wood using hand saw without using hand protection.
3. Majority of the workers were not wearing eye protection.
4. Mr. Badelaziz Yousuf was wearing a defective/worn out safety shoes.
Recommendation 1

Gloves shall be selected and worn based on the materials or equipment being handled. Gloves shall be resistant to hazards specific to the
job. (Reference: CSM I-3; Section 3.6.1)
At a minimum, hard hats, safety glasses, and safety shoes shall be worn in SA restricted areas and project work sites. (Reference: CSM I-3;
Section 3.3.2)
PPE shall be visually inspected by the user before each use. Defective or damaged PPE shall not be used and shall be immediately
replaced. (Reference: CSM I-3; Section 3.3.7)

Chemicals without SDS:
1. 714 CPVC Plastic Pipe Cement (at ROTA)
2. Hempel Curing Agent 95881 (at ROTA)
3. SIPSEAL 220 (at ROTA Area)
4. SIPOXY Shield 240 (at ROTA Area)
5. Tarcoat BG (at ROTA Area)

Complete and current MSDSs/CHBs for each chemical or hazardous substance stored/used on site shall be readily accessible (e.g., at the
location of the material storage or handling areas) to employees and contractors and to emergency response personnel (Reference: CSM
I-10; Section 10.5.1)

Finding 5
No storage area was allocated for chemicals used at site, chemical containers were noted scattered around site under the heat of the sun.

Specific storage and handling recommendations for hazardous materials provided in MSDSs and/or CHBs shall be followed. (Reference:
CSM I-10; Section 10.6.3)
Hazardous materials shall be protected against shock, incidental mixing with other materials, damage to containers, undue heat from the
sun or other sources. (Reference: CSM I-10; Section 10.6.1)

Finding 6
At Reverse Osmosis Tank Area
Hand saw being utilized by a worker to cut a piece of wood noted with defective handle, wooden handle was tied with galvanized wire &
"U" bolt.

Excessively worn, defective or deformed tools shall not be used. If excessive wear, defect or damage is observed, the tool shall be
immediately tagged and withdrawn from use for repair or disposal. (Reference: CSM I-11; Section 11.3.7)

Finding 7
At Sewage Treatment Plant Area
Two (2) safety harnesses noted with non-self locking carabiners.
Recommendation 1

Snap hooks and carabiners shall be self-closing and self-locking. (Reference: CSM II-5; Section 5.5.7)

Finding 8
At ROTA & STP Area.
No Reporting Instructions were posted at site.
Recommendation 1

Signs shall be conspicuously posted at the work site with the emergency telephone numbers and reporting instructions. (Reference: CSM

Contractor's site supervision and safety staff shall be trained in, but not be limited to, the following :
*The job-specific CSSP and HIP.
*The contract's safety requirements.
*Specific hazards and safe work practices associated with the job.
*Relevant sections of the SA Safety Handbook and the SA Construction
*Safety Manual (CSM).
*Injury/incident reporting and investigation.
*Applicable emergency response procedures.
*First aid and basic life support (BLS). Note: They shall have valid first aid and BLS certificates in their possession at all times.
