    1. Stop work until wearing full body harness.
    2. Training was conducted for safe work at heights.

    Root Cause Analysis


    01. Lack of barriers.
    02. Lack of Supervision.
    03. Lack of knowledge

    Preventive Action:
    01. In complete should be red tagged.
    02. The scaffolding inspector shall place the green tag to use after the completion of the
    scaffold.
    03. Personnel shall not be permitted to work if scaffolding is in complete.
    04. Periodical inspection shall be carried out by competent scaffolding inspector.
