SCOPE (List whom this procedure applies to and the specific location this work can be conducted in)
1. Plant Supervisor/Asst. Supervisor – Overall Supervision of the Plant
2. Duty Engineer – Commands and assign works to Technicians
3. Safety Officer – Ensures Safety and Health in the work place is enforced
4. Technicians – Field workers assigned in the activities
5. Driver – Assist the Field Workers on the Transportation
6. Warehouse Personnel – Provides materials needed for the site
Location: String Combiner Box Area from Zone 1 to Zone 8
AUTHORIZATIONS: (List specific operator competency requirements, e.g. area induction, qualifications, certificates, OHS
training, supervision. List who can approve that competency has been achieved)
1. Mandatory Eight (8) – hour OSH Orientation
2. National Certification (NC II) which grants personnel to become qualified and to work
on specific job description (e.g. Electrical works – Electrical Certification)
3. Authorized by Gigasol Palauig
4. With Fit-To-Work clearance from physician
HAZARDS: (List all the potential hazards and associated consequence, e.g. chemical exposure – inhalation or skin
absorption, leading to irritation, burns, acute or chronic injury).
1. Fire: burns
2. Physical: contact with sharp edges of tools and equipment. Tripping from loose rocks
and tree branches
3. Ergonomic: awkward and repetitive motion can cause strain to muscle and joints of the
body
4. Biological: Insect and snake bites
5. Weather: extreme heat index and humidity can lead to heat stress and dehydration;
thunderstorms
6. Gravity: Fall from slopes; falling branches and other objects
7. Electricity: contact with energized DC Cable Strings and SCB Equipment
8. Psycho-social hazard: fatigue; ineffective communication

EMERGENCY (Emergency response procedures e.g. power isolation procedures, first aid response)
PROCEDURES: In case of injured worker
1. Activate medical assistance
2. Apply first aid and secure injured worker
3. Transport injured worker to the Site HSE Office for further evaluation and possible
referral
4. Refer injured worker to nearest hospital for medical/ surgical intervention
5. Prepare and submit incident report in compliance with Gigasol Palauig policies
