Introduction:

The health & safety inspection was carried out at Barmer, Rajasthan on 29 th February 2016, where
Azimut Energy Services is carrying out 3D land seismic survey activities for their client Cairn India
Limited. A full-fledged base camp has been set up for the work to be undertaken. Operations for 3D
land seismic survey activities are being undertaken on daily basis with a crew strength of about 1000.
The activities start early in the morning at about 04:30 and complete at 19:00 hrs when all the vehicles
are parked safely in the earmarked parking area. The inspection was carried out covering the base
camp. accommodation, catering facilities, clinic, offices, workshops. There are three kitchens being
maintained one for expats, one for senior Indian staff and one for junior Indian staff. The total base
camp area spread is about 800X500 square Meters. Seismic survey equipment like 6 vibro trucks, one
up hole rig, recorder remain in the field. In addition, about 200 vehicles have been hired for crew to be
taken to the field and bring them back. Four generators (3 phase) of 250 KVA ratings are installed and
are being operated 2 at a time for 12 hours each to provide independent power generation & distribution
for the entire base camp. Water supply for the base camp was taken from local source, for the purpose
of washing & bathing. The drinking water being provided through packaged bottling and a standalone
RO plant for base camp drinking & food preparation. The base camp is also well protected by having
fencing all around. A service provider has been entrusted with providing base camp & field security
personnel.

The operations are being carried out by using 80% of labour from local area. The base camp was
shifted three times to cover project area of 2500 square kilometres. A total of 6000 inductions were
carried out at the time of inspection. During inspection the entire day’s activities were covered right from
the first TBT which was conducted at 04:30 Hrs in the morning, till the last vehicle was parked inside the
camp at 19:30 Hrs.

There was a full HSE team led by one lead HSE advisor and 5 HSE supervisors to take care of HSE
related activities and maintain safety culture for the project.

Executive Summary

This inspection report carried out on 29th February 2016 outlines & highlights main 6 areas where
management attention and follow up for improvement is required. The observations documented are
events observed during inspection, giving possible consequences and suggested control measures
which could address the risk associated with unsafe conditions or unsafe acts. General training was
observed to be of a very high standard and well imparted for monitoring local labour which was
changed from base camp to base camp in line with CSR requirements of client Cairn India Limited
policy. A detailed coverage, which is an outcome of this inspection, is provided below with
approximate costs and timescales for implementation.

Fire Hazard

The area of operation is having high temperature throughout the year and remains dry most of the time
hence, the risk of fire is very high. Hence, proper waste management, electrical grounding, designated
smoke area, monitoring & supervision of no smoking in the tents & bunks is mandatory. For firefighting
purpose provision of fire extinguishers, smoke detectors, fire station & fire truck was made and facilities
were well maintained and in place, at the time of inspection

Electrical Safety

Four 250 KVA three phase diesel generators were installed for required power generation & distribution.
Satisfactory safe and secure power distribution was in place and all electrical cables were underground
and safety of ELCBs, RCCBs & MCBs were provided at distribution & junction boxes. However, the new
accommodation was not covered by the electrical power & distribution standard wherein there were
number of improvements which were considered mandatory and they were brought out through
observations. A third party electrical audit was undertaken initially when base camp was established,
however not for the additional accommodation.

Machine/Vehicle Safety

Though equipment maintenance and monitoring of operations was in place there was a room for
improvement in transport department specially when about 200 vehicles were being utilised for
transportation of the crew from base camp to field & back.

Legal Compliance

The gas cylinders’ storage requirement not to exceed 100 kg is mandatory and if gas license is not
obtained then the storage limit for full gas cylinders is approximately 5 LPG cylinders (100 Kg), as per
gas rules 2004 (PESO). This legal compliance is strictly to be maintained as per PESO regulations in
India.

Biomedical waste requires to be disposed of as per the bio medical waste disposal rules and autoclave
requirement is mandatory to ensure that,

Working Practice

Refresher training & campaigns required to be undertaken as there is a continuous out flow & inflow of
labour work force due to CSR policy of the client Cairn India Limited. The working practices such as
manual handling (as major portion of operation is labour intensive) the work force needs to be well
aware of unsafe conditions & unsafe acts during manual handling.

Work Environment Requirements (Specially for rest considerations)

Ventilation in workshop and air conditioning in some of living accommodations was not sufficient, Air
conditioners’ maintenance requires a special focus and to be undertaken on priority more so with high
temperature prevalent conditions in the local area. Also in new accommodation for the labour work
force in the tents, only one pedestal fan was provided, which is considered to be inadequate. One more
additional pedestal fan in each tent to make things comfortable for the workers inside the tent in hot &
dry conditions, considering occupancy strength & area of each tent.

Welfare

There is noticeable lack of recreation facilities, in the base camp. There is a need to provide sheds,
sport places and restrooms which will help to ensure fatigue management. It will certainly lead to
maintain positive morale of the work force.

Main Findings of inspection

Fire Hazard:

During the inspection a number of fire hazards identified as improper waste management, waste
segregation area, batteries charging workshop (overload on work force) and inadequate grounding and
potential static accumulation; the storage of gas cylinders; smoking in not designated places are some
of the factors which need to be monitored & controlled. Overall, the monitoring & control through
inspections is considered satisfactory. The fire drill is conducted with a frequency of once in a month
which is followed up with a drill report to the client and findings & recommendations are taken up by the
responsible persons for closing out, through ATR (Action Tracking Register). However, scrutiny of ATR
reveals that the follow up actions have not been carried out as per the scheduled target dates. There is
a gap in closing out of action points which needs to be addressed by the operational team, is not
complied with as per agreed schedule. The logistic support positioning has inherent delay hence
process of requisition, identification of items to be purchased, placing of order & positioning the material
needs to be expedited and procedure to be put in place for timely completion of outstanding action
points. The IRT (Incident Response Team) is competent to carry out the incident response. The
frequency of the refresher training for IRT may be looked into and streamlined.

Head of battery charging workshop to ensure charging SOP in place, appropriate chargers provided
and keep more focus on personnel to ensure following of company’s policies and best practices.
Additional manpower for battery workshop needs to be inducted, trained & deployed to reduce workload
on battery workshop staff.

Designated smoking areas in camp and field to be identified and communicated to all employees.

Electrical Safety:

During inspection a few hazards and activities observed where electrical hazard’s controls were
missing. Additional accommodation electrical load & distribution to be worked out and balancing and
safety to be ensured for the camp. Undergrounding of cables is mandatory. Appropriate number of
ELCBs, RCCBs & MCBs to be provided after complete load calculations & distribution for the electrical
power. An electrical audit from a qualified electrical engineer is also mandatory. The IP ratings for all
the electrical equipment which are subjected to outside weather and located outside offices &
accommodation like, distribution panels, pole lights switch, lamps be mentioned and should be available
during inspection.

Effective bonding and earthing of all metallic equipment also to be checked during weekly inspection as
a measure of defence against lightning strikes. Finally, regular inspection of all bonding and
measurement of earth loop impedance is necessary. With additional cabling for new accommodation
earth point impedance is to be reconfirmed during proposed audit. All joints be re-inspected and redone
with documentation of each stage pictures, as it’s mandatory to establish safety of each joint. Wherever
possible it’s better to avoid joints and may be replaced by continuous cable from point to point.

Estimated immediate cost to implement these measures is around INR 2,50,000/- & additional
administration cost.

Machine/Vehicle/Equipment Safety:

Vehicle is considered to be the biggest hazard for this project hence control measures applied for this
activity have to be suitable & sufficient for maintaining safe outcome. Though the process of vehicle
selection & driver and defensive driving to the drivers is in place, the vehicles being utilised need to be
properly controlled for the scheduled maintenance. It was observed that many vehicles were overdue
for scheduled maintenance and operational pressure was not allowing transport manager to spare
vehicles in time. There was no tagging system to identify whether vehicle is due for servicing or not.
Hence a proper monitoring & identifying system for vehicles to ensure that every vehicle which is due
for maintenance is identified & released for scheduled maintenance work to the authorised service
centre from the OEM. On return all the serviceability checks to be carried out by the transport manager
to ensure that the maintenance work is carried out completely and correctly. A record to be maintained
of these checks and deviation if any to be pointed out and corrective action to be taken for the same. In
addition, all the vehicles are required to carry basic towing equipment and a shovel as accessories to
cater for any breakdown in the field.

As regards to kitchen equipment, it was found to have no mechanical guard for moving parts. The
equipment not to be used till the mechanical guard is provided to avoid direct contact of body with the
moving part of the machinery.

Legal Compliance

As regards to gas storage, continuing the same practices will lead to violation of the legal requirements
of Indian Gas Cylinders Rules, 2004, which can cause punishment under Indian law. Without having
gas license (which requires an elaborate arrangement such as building a safe storage space as per the
approved engineering drawing from PESO inspector) the maximum number of filled LPG gas cylinders
is restricted to 100 Kg of LPG gas which approximately works out to be 5 LPG gas cylinders. There
needs to be a proper process to ensure this legal requirement and quality control & quality assurance
checks need to be instituted in the procedure to ensure there is no deviation from the process. There
could be an additional check at the gate by security wherein number of trips of vehicle carrying LPG gas
cylinder could be monitored and only 5 LPG cylinders could be allowed by giving gate pass, thus

avoiding entry of additional gas cylinders inside the camp. The storage place could be checked through
daily inspections to ensure that this legal requirement is strictly complied with.

The other legal compliance is biomedical waste disposal. Due to the size of the camp there is certain
amount of biomedical waste which is generated. The needle (medical sharps) electrical incinerator is in
place but there is no autoclave as the one held earlier is repairable and handed over to service centre
for repair. For last month or so the autoclave has not been brought back after repair. If repair is not
possible then a new suitable autoclave needs to be purchased & positioned in the clinic for sterilization
purpose. The biomedical waste is accumulated for a week and then disposed of through an authorised
disposal agency, which is not an acceptable practice as per the biomedical waste disposal rules.
Priority attention is to be given to both these legal compliance issues and to be resolved at the earliest
opportunity.

Working Practice

There are numerous unsafe behavior/practices observed, including workshop workers showing lack of
using manual handling, safe techniques and not wearing appropriate PPE while handling batteries and
geophones, not using proper secondary containment/drip trays during fuelling. Repair of bunding for the
generators also to be taken up on priority. Manual handling requires special attention as the labour work
force is constantly replaced during the execution of this project over a wide area of entire district of
Barmer, Rajasthan.

Ergonomic

A few ergonomic hazards observed such as an inadequate temperature in living rooms, poor lighting
near workshop area, ventilation in batteries charging room is inadequate. These findings can be
controlled by request to sub-contractor to replace old ACs (Air Conditioners) with new ones as per
subcontract, so there is no additional cost required. Work station chairs were observed to be in
unsatisfactory condition and were not maintained properly. It was causing restriction in posture and
operator was frequently adjusting having continuous avoidable movements which may result in fatigue.
The awkward postures are considered unwarranted and need to be corrected at the earliest
opportunity. These conditions could cause long term effects on body and considered to be dangerous
as they are not appreciable by the operator in a short duration of exposure. There is a requirement to
monitor & control these effects till the time it is done the operator may resort to take frequent short rest
breaks rather than infrequent longer ones. They could avoid sharp increase in work rate. Work load
needs to be gradual enough to ensure that it does not result in excessive fatigue.

Welfare Facilities

The long hours of work routine, harsh environment and continuous production activities mean that work
force is always busy in carrying out assigned tasks. Once they come back to base camp after hard
day’s work, recreational sports are important to the quality of life of local communities. It would certainly
provide benefits to health such as prevention of heart disease & diabetes. It also helps decrease
overweight and obesity numbers with regular physical activity among people and communities. With
hard day’s work it’s important to destress with these activities as they create positive outcomes such as
confidence, self-respect, self-esteem, trust, self-reliance and leadership abilities. For a very long project
spread over an area of 10kmX10km field area and duration of over 2 years, we definitely need to look
into overall development of people which will make lives of labour healthy and it would also be in the
interest of social responsibility.

As regards to workers in camp, the workshop workers have direct exposure to the sun during the day
and have no place to have rest and refresh themselves. Also, there is no recreation facility available in
camp for junior staff to be able to relax after work. These are destressing activities and are mandatory.
Not following up on this matter will lead to low morale in workers. There is an immediate requirement of
organizing recreation facility such as volleyball, gym and table tennis. The team games are ideal for
developing a team work culture and healthy competition within the organization.

Conclusions:

In general, the seismic crew has achieved a good production time having good level of HSE controls in
place for most of the risks. Still there is a room for improvement and controls in terms of fire hazards,
electrical safety, ergonomic, working practice and welfare to be implemented. The major & significant
findings identified in this report can cause legal action by enforcement department and/or accident.
These findings need to be followed up by providing written SOPs (Safe Operation Procedures),
trainings, TBT (tool box talks) and better supervision, which is very effective administration controls with
low cost involvement. The main process of information, instructions, training & supervision needs to be
followed up.

The implementation of recommended improvements will eliminate the possible legal actions which
might be applied to company causing project delays, reputation impact due to breaches of Indian law
and reduce the potential of accident by having appropriate level of controls in place and keep high level
of safety awareness throughout the seismic crew to ensure smooth operations.

Barring from a few mandatory requirements, a number of recommended actions require no financial
cost. The important observations can’t be addressed without additional resources and costs. It is
pertinent to mention at this juncture that the cost of consequences in the event of non-adherence to
these recommendations, is many times more than the financial burden the company would have to bear
with. Moreover, the cost is negligible compare to size and spread of the very big 3D land seismic
project, it would work out to be just 1/3 of daily expenditure being incurred for this project by the
company management
