. Storage of diesel beside
generator.
. Smoking in record room.
. Paper material stocked in
dustbin after use.
Risk: May cause fire and burn to
people and may damage to
property.

4. Poor Work Environment: No


adequate lighting in record room.
Risk: May cause collision with
stationery cabins, tripping and
falling.
. Not proper ventilation in record
room.
Risk: May cause biological
contamination and suffocation.
. Not ambient temperature, in
this season low temperature.
Risk: May cause fever and flu in
cold season, also stress.

4. Violence: Due to public


dealing department there is face
to face contact between
employees and workers.
. Public waiting at same place
where employees are working.
Risk: It may cause violence and
stress to the employees.

5. Slip & trip: Due to lack of


maintenance Water dropping
from water dispenser on slippery
floor.
Risk: May cause slips and fall,
injury.
. In the photocopy machine cabin
trailing wire between cabin and
machine.
. Risk: May cause trip and fall
injury to people.

6. Ergonomic: Officers chair


more high than table required.
Risk: Musculoskeletal disorder,
back pain.

7. Poor housekeeping: Broken


masonry pieces present due to
repairing work in office.
Risk: May cause fall due to slip
and trip.

8. Dust: On 2nd floor near stair


passage crash, sand and cement
present and causing dust.
Risk: May cause lungs problem,
asthma and eye irritation.
9. Computer work station:
Working on computer without
screen protector.
Risk: Eye irritation, stress and
fatigue due overload of work.

10. Emergency evacuation route


obstructed by broken non-
useable chairs.
Risk: May cause tripping and
injury in emergency.
