Executive Summary

Date of review: 04/06/2013

The inspection found that there are few numbers of good health and safety practices in the work
place and is really appreciable. However, apart from these report some of six issues reported in
the inspection, which required improvement and corrective actions are listed below.
1. A high risk of overturning of stacked tiles due to its over height. Workers stacking tiles
vertically without any side support. This will lead to serious injury or fatal to workers and loss of
damaged product. The over height of the stacking should be reduce without 1.5 meter.
2. A high risk of fall of pedestrian and overturning of vehicle into the excavation. The excavation
is placed near the turning of vehicle route. It causes death and damage in the company.
Provide adequate hard barricade, signage and warning.
3. Routes of pedestrian and vehicles are not segregated in the work place. High risk of collision
of vehicle with pedestrian, it causes injury or death. Separation of vehicle and pedestrian route
is necessary to prevent accident and related cost.
4. There is a fire risk in the flammable liquid stored area, because the area is close to kitchen.
Fire cause loss of life and property. It will reduce the reputation of the company. Remove
flammable liquids immediately from the area keep securely.
5. A high risk of electric hazard in electric blower. Using substandard electric joint to extend the
length of the cable. Insulation of the cable is damaged. As a result, there is a risk of
electrocution and damage to equipment. So, provide standard double insulated electric
components in the work site.
6. First-aid facilities are insufficient in the work place. First aid box have no suitable medicines
and materials related to the work. It will not meet the aims of first-aid. Risk of deterioration and
delay of recovery from injury. It is a legally bounded issue. Provide first-aid room and suitable
medicines in the FAB.

Main findings of the inspection

Observation: 1
Substandard electric joint using to extend the length of the
electric blower cable and it also damaged insulation. Electrocution
and short circuit causes fatal and damage to equipment.
Introduce standard electric fittings and cable joint kits will
minimize the risk accident.
All the electrical work has to be accordance with ILO Convention C
152(1979)
The financial implication for purchasing standard electric joint kit is
approximately RS 1000
Observation: 5
Interlocking tiles are stacked over than the average height of a man. A high
risk of overturning of tiles and that causes serious injury and death, damage to
equipment.
To prevent the overturning of the stacked tiles rearrange the height as
shoulder level. C152 - Occupational Safety and Health, Convention, 1979
(No. 152) says the safety in work place. There is only loss of production
time.
Observation:16
There is a high risk of fire related to the storage of flammable liquids near the
kitchen. Keep the flammable liquids from the presence of fire to prevent fire in the
workplace. Isolate the storage of flammables with barricade.
All these storage facilities should be accordance with ILO convention R175
(1988).
The financial implication for the control measure may be maximum RS 3000

Observation: 18

There is no separate route for vehicles and pedestrian in the workplace. A high
risk of collision of vehicle with pedestrian causes serious injury or death.
To prevent the accident, segregate the routes of pedestrian and vehicle. The
accidents in industrial work site are accordance with ILO Convention C174 Prevention of Major Industrial Accidents Convention, 1993 (No. 174).
The cost for fulfilling the control measure approximately is RS 8000
Observation: 21
Dangerous condition of the excavation, which is situated near the side of road side.
There is a high risk of fall of pedestrian and overturning of vehicles into the excavation.
To prevent fall and overturning, provide adequate barricade, warnings and sign
boards.
The financial expense for the control measure is approximately RS.2000

Conclusion

There are many good practices in the company, but from the point of observation in
the inspection, it is assumed that the organization maintains unacceptable limits of
occupational health and safety standards; however the above observations and its
recommendations are to be taken in account by the management.
The recommended findings in the report are;
Usage of standard electric fitting.
Reduce the over height of stacked tiles.
Safe storage of flammable liquids from fire.
Segregate the routes of pedestrian and vehicles.
Adequate hard barricade, warning and signals for excavation.
These points are the main issues in the company, it has to be resolved in the
recommend time period to avoid obligation and breach of legal requirements in accordance
with International Labor Organisation. If you dont mind the recommendations as time
bounded, it may cause to further enforcement action, accident and it also effect the
reputation of the company, employees morale, production process etc. The cost related to
an accident is very high as well as compensation to workers, court procedures etc. I am
submitting the report with positive expectations from your early actions and really
appreciable.
