Preliminary Details of Incident


Whilst conducting blasting operations in the inspection shed the IP was manually transporting a plastic end cap from one
pipe to another. An empty cardboard box was on the floor of the worksite and the IP tripped over this and fell to
ground. Upon landing the IP struck head against the end cap that the IP was holding. The IP received first aid onsite and
appeared to have an approximately 2-inch
laceration to the front right temple.

What Went Wrong?


A cardboard box was incorrectly located on the
floor of the worksite.

The IP had been completing a separate working


task (filling the grit blasting machine) and had
failed to remove the packaging materials (box and
pallet) from the worksite prior to progressing with
the next working task.

The IP did not regard the materials on the floor and the lack of appropriate housekeeping as a safety issue as this is a
regular poor working practice in this area and showed poor safety hazard awareness.

The worksite has trip hazards which have not been identified and appropriately managed as to eliminate the risks to
personnel.

Immediate Cause(s)-
Failure to identify the tripping hazard by the operator due to complacent working practices.

Immediate Actions-
- Toolbox talk in the incident was conducted with all personnel.
- Risk assessment and safe system of work was reviewed and updated.
- Training was conducted on good housekeeping to all personnel.
- A storage area for materials was created and marked accordingly to ensure that materials are not left in high
volume footfall areas.
- A site inspection was conducted to identify further HSE risks to personnel.
