Description of Accident
In this incident, a worker was found dead on the ground floor of a construction site.
Preliminary investigations revealed that the worker was carrying out plastering works at the
balcony area within a unit on the 17 th storey.  The worker was found without any fall
protection equipment and it was suspected that the worker fell from the edge of the balcony
and landed on the ground. The guardrails that were installed were dislodged at one end.

 A similar incident had occurred earlier this year. A worker carrying out plastering work on
the external wall of an air conditioning ledge fell to his death from the 9 th floor of a building.

Provide at least SIX (6) Basic Causes of accident.

1. The worker did not go for Work at Height (WAH) WSH Training.
2. Workers did not use Personal Protective Equipment.
3. Guardrails were dislodged at one end.
4. Unsafe Work environment
5. Lack of Safe Work Procedure (SWP).
6. Lack of proper supervision.

Provide ONE Root Cause of the accident and justify with reason.

The Worker does not have adequate training when working at a height. The worker does not know the
importance of PPE when working at height. Thus, neglecting the importance of PPE. The worker also
does not know the dangers of working at height and what preventive measures they can do.

Recommendations to at least SIX (6) corrective/preventive measures:

1. Ensure there is a supervisor supervising the work.


2. Ensure the worker uses personal protective equipment.
3. Ensure there are guard-railings, and it is working well.
4. Ensure the workers go for the WSH Work at Height training before working at height.
5. Ensure there are safe work procedures done to ensure safety.

6. Ensure good housekeeping and no lying objects on the floor.


 Group Activity 2: Assessing WSH hazards and risks
 Identify FOUR (4) different key activities that is related to the Incident Case to evaluate the
impact of safety and health of persons exposed to hazards. Each key activity should have at
least TWO (2) hazards. Present your findings on a RA form using the 5 x 5 risk matrix.
