Inspection Report

1. Purpose and objectives for the safety inspection

Safety inspections helps to prevent incidents, injuries, and illnesses.

Safety inspections helps to identify and record hazards for corrective action.

Safety inspections enables HSW committees better understanding of task and work
procedures.

Safety inspections also enables workers and supervisor to share their concern at
workplace.

It helps HSW committees to better eliminate hazards and control the risk in monitor
step.

Lastly, once the inspection report was generated after the analysing the evidence and
interview with workers. The daily briefing and preventive action must apply.

Description of Unsafe Condition/Unsafe Act:

Poor Housekeeping at site

Possible reasons for the unsafe condition/unsafe act (Root Causes):

Plan to clean up on the completed date

Lack of manpower

Preventive Actions:

Supervisor needs conduct ‘toolbox’ meeting before work.

Workers need to submit photos once they complete the housekeeping.

Description of Unsafe Condition/Unsafe Act:

The worker was not wearing safety shoes.

Possible reasons for the unsafe condition/unsafe act (Root Causes):

They unaware of the hazards of not wearing their personal protection equipment.

It may be uncomfortable or wrong Size when wear the safety shoes.

Preventive Actions:

Stop his work immediately and issues warning letter

Conduction daily “Toolbox” meeting and provide proper PPE for work

Water leaking from the water pipe and the wire is on the floor

Possible reasons for the unsafe condition/unsafe act (Root Causes):

Poor workmanship and forgot to turn off the main switch and water riser

Preventive Actions:

Assign HDB approved electrical and plumbing specialist for the work

Issues “Lock-out “Master Tag and displays “Danger” signboard before work.

Inappropriate set up of scaffolding by the contractor

Possible reasons for the unsafe condition/unsafe act (Root Causes):

Insufficient of equipment and tools to set up

Save cost

Assign approved scaffolding contractor.

Display signboard to warn others.

Description of Unsafe Condition/Unsafe Act:

Stepladder was used folded up and leaning against a pillar.

The worker was lean off the side of a ladder while working.

Possible reasons for the unsafe condition/unsafe act (Root Causes):

The worker was not well trained for the work at height course.

The proper stepladder was inadequate.

Preventive Actions:

Apply bubby system (2 workers) to work when use the stepladder.

Request for scaffolding if the ladder cannot reach the height.

4. Recommendations to prevent the repeat of the similar findings:

1) Supervisors must conduct “Toolbox” meeting daily before work starts to remind the
workers to do housekeeping.

2)Supervisors must ensure the assigned contractors and workers with required permit
and certificate for the work.

3)Assign safety officers to patrol and identify any unsafe act and unsafe workplace in
site.

4) Record the hazard and re-inspection the findings.
