Description of Unsafe Condition/Unsafe Act:

A worker not use safety goggle while performing Cutting works

Possible reasons for the unsafe condition/unsafe act (Root Causes):

Worker’s safety goggle has broken

Preventive Actions:

To provide new safety goggle to the worker and inform him to use safety
goggle while performing cutting works
To rectify by: In 2 hours (By Today) Action by: Supervisor Santhosh

Description of Unsafe Condition/Unsafe Act:

No fire blanket nor fire extinguisher provided while performing

welding works Possible reasons for the unsafe condition/unsafe act

(Root Causes): Worker forget to bring fire extinguisher and fire

blanket to hot work area. Preventive Actions:

Worker must be informed to standby a fire extinguisher and fire

blanket while performing welding work/hot works.

Supervisor must only allow welding work to be carried out after assessing
the safety requirements needed to perform those works

To rectify by: In 1 hour Action by: Supervisor Swami

Supervisor must request adequate portable ventilation fan from

management before caring out work in confine space

Poor Housekeeping at 1st floor which may cause slip and fall down

hazard Possible reasons for the unsafe condition/unsafe act

(Root Causes):

Lack of supervision from supervisor especially at the end of the work shift

to ensure housekeeping was done properly

Preventive Actions:

Supervisor need to ensure housekeeping is done properly after the end of the
work

No signage or barricade provided to inform that there is a floor opening.

Possible reasons for the unsafe condition/unsafe act (Root Causes):

Lack of supervision from supervisor

Preventive Actions:

Supervisor must ensure barricade and signage is provided at the floor

opening area to inform other working personnel that there is a floor opening
at that area.

4. Recommendations to prevent the repeat of the similar findings:

● Supervisor must ensure that worker have adequate PPE and ensure
that they use the PPE while performing work

● Supervisor must carry out work only when the safety requirements
stated in the permit to work/risk assessment are satisfied.

● Supervisor must check the workplace before leaving the workplace once
worker have performed the job.

● Remind to all workforce during daily toolbox meeting about PPE and
proper housekeeping.
