On August 13th, 2022 time 09:00 AM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers did not wear special PPE (full-
body harness) when working at heights, potentially
causing workers to fall down and get serious
injuries
Location: Outbound (permanent staircase #6)

On August 13th, 2022 time 09:01 AM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers who were installing rainwater
down pipe (RWDP) using scaffolding that was not
proper and not checked/verified/tagged (green) by
the certified scaffolding inspector

On August 13th, 2022 time 09:48 AM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers who were installing the ceiling
frame were not wearing full-body harness leg
straps properly and were not wearing safety shoes
and using scaffolding that was not proper and not
checked/verified/tagged (green) by the certified
scaffolding inspector

On August 13th, 2022 time 10:00 AM while BVI-
HSE Performed daily site inspection, found unsafe
condition the dismantled formwork material was
not immediately taken down, potentially fall down
and hit the workers below

Description: Description :
On August 13th, 2022 time 10:35 AM while BVI-
HSE Performed daily site inspection, found unsafe
condition there was a gap in the steel plate that
was used as temporary access, potentially causing
workers to fall into the hole
Location: In front of the utility area

Description: Description :
On August 13th, 2022 time 10:50 AM while BVI-
HSE Performed daily site inspection, found unsafe
condition the protruding rebar was not cut
immediately, potentially causing someone to get
caught by the rebar
