    On June 18th, 2022, an inspection of the Bogor E-Commerce Hub project found several unsafe conditions and actions. Six findings were recorded, including steel materials blocking access and not arranged properly, temporary electrical panels hung insecurely from scaffolding without safety documentation, workers standing outside scaffolding guardrails without barricades during steel erection, improper welding machine power cable connections, unplugged power cables, and a damaged webbing sling used for lifting.     Performed daily site inspection, found unsafe
    condition the placement of steel structure material
    blocked access and was not arranged properly

    On June 18th, 2022 time 09:17 AM while BVI-HSE
    Performed daily site inspection, found unsafe
    condition the placement of temporary electrical
    panels was only hung on the scaffolding and there
    was no safety sign, PIC, single line diagram posted
    on the panel, also there was no fire extinguisher
    provided

    tape or hard barricade installed around the work
    area as a warning to other workers

    socket was not equipped with a plug, potentially
    causing electrical short circuits and fire

    Performed daily site inspection, found unsafe
    condition the webbing sling used to lift formwork
    material using a mobile crane was damaged,
    potentially causing the webbing sling to be cut off
    and the material being lifted to fall down
