On June 23rd, 2022 time 10:00 AM while BVI-HSE
Performed daily site inspection, found unsafe
action PIBK workers did not wear special personal
protective equipment (full-body harness) when
working on scaffolding and also use scaffolding
that was not proper and also was not checked/
verified/tagged (green) by the certified scaffolding
inspector

Performed daily site inspection, found unsafe
condition the electrical panel casing was damaged,
potentially causing someone to be electrocuted

condition the installation of the steel column was
not fixed properly, potentially causing the steel
column to collapse

On June 23rd, 2022 time 16:00 PM while BVI-HSE
Performed daily site inspection, found unsafe
action PIBK workers who were casting parapet
walls did not wear special PPE (full-body harness),
potentially causing workers to fall down and get
serious injuries
