    Location of Incident: FSA AHU-207/208 room at Level 2 Date of Incident: 25/07/2022

    Time of Incident / Discovery: 1430hrs

    Nature of Incident & Details:

    1410hrs – Mech TO is working on the Preventive Maintenance work at FSA-207 AHU-2 for
    replacement of filters.

    1430hrs – During the replacement of filter works, he tripped over by the AHU drainage pipe and cause
    himself to fell onto the ground.

    1530hrs – After an hour later, Mech TO realized his right-hand wrist starting to swollen.

    1635hrs – Mech TO reported his condition of his incident to Assist Mech TO’s Rozaimi.

    1635hrs – Assist Mech TO informed Site Manager’s Eric Tay on Mech TO incident and immediate
    send Mech TO to see Doctor.

    1819hrs - Site Manager called Mech TO on his condition and status. Mech TO replied that the clinic
    operation hour is from 1900hrs onwards and will update later.

    1937hrs – Mech TO called Site Manager that the clinic’s Doctor refer him to Ng Teng Fong General
    Hospital.

    2032hrs - Mech TO arrival Ng Teng Fong A&E.

    0218hrs - Mech TO was in Emergency Department from 25/07/2022 2032hrs to 26/07/2022 0218hrs.
    He is awarded Hospitalisation Leave from 26/07/2022 to 01/08/2022, due to his right wrist fracture
    accordingly to the Ng Teng Fong General Hospital diagnosis report.
