On August 15th, 2022 time 07:54 AM while BVI-
HSE Performed daily site inspection, found unsafe
condition many piles of garbage that were not
immediately removed

On August 15th, 2022 time 08:15 AM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers only use SHS (square hollow
section) steel as a platform for access and also
there were no horizontal and vertical lifelines
installed in the area

Description: Description :
On August 15th, 2022 time 09:21 AM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers who were installing ceilings
did not wear special PPE (full-body harness) and
also use scaffolding that was not proper and was
not checked/verified/tagged (green) by the certified
scaffolding inspector

On August 15th, 2022 time 09:45 AM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers who were painting columns
did not wear PPE (full-body harness and safety
shoes) when working on scaffolding, potentially
causing workers to fall down and get serious
injuries

On August 15th, 2022 time 15:00 PM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers who were installing steel
columns using scaffolding that was not proper and
was not checked/verified/tagged (green) by the
certified scaffolding inspector

Description: Description :
On August 15th, 2022 time 15:30 PM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers who were doing hot work
(welding) did not wear special PPE (apron, welding
gloves, welding masks) and also did not provide
fire blankets to protect sparks that fall down and
there were no vertical and horizontal lifelines
installed in the area which functions as an anchor
point when workers want to move around
