On January 20th, 2023 time 09:12 AM while BVI- HSE
Performed daily site inspection, found unsafe condition
the power cable connection on the temporary electrical
panel was not properly protected and also the MCB
installation on the panel was not installed properly and
there was no fire extinguisher provided next to the
panel

the protruding rebar was not cut
immediately, potentially causing someone to
stumble
