EXECUTIVE SUMMARY

1. Incident Briefing – Asbestos Cement A/C Water pipe

On 5th of January 2024, Wawasan Dengkil was carrying out works to backfill ground beam at
Station 14 Entrance 2 to be handed over to Wai Tat for RC works. The work was carried out by
the operator based on instructions given by Mr. Lee Jih Way, WD Supervisor in charge of
Station 14 Entrance 2. At around 1:50 pm, The excavator bucket hit the Asbestos Cement (A/C)
Water Pipe besides Federal Highway at the depth of 1 meter while carrying out soil excavation
for backfilling of the ground beam which has already been casted. The strike caused damage to
the Asbestos Cement water pipe and causing it to leak. This is due to no proper communication
between the Operator and PIC on the works to be carried out. When told to backfill the operator
carried out soil excavation works to backfill the said area instead of using the soil provided by
WD for backfilling works. The work was immediately stopped, and the incident was reported by
WD Supervisor to WD Safety and Health Department. The incident was then notified to Air
Selangor by WD Project Manager. Air Selangor then sent their appointed Contractor to site for
further investigation and rectification works. Air Selangor appointed contractor arrived on site at
2000 hours on 5th January 2024 to investigate regarding the incident and immediately carried out
rectifications works accompanied by WD PIC on site. Damaged Asbestos Cement A/C water
pipe rectified at 0500 on the 6th of January 2024.

2. Initial Findings

i) Daily Toolbox and Pre-Task Briefing was carried out prior to commencement of work.

ii) PTW and supervision by PIC was available.

iii) Poor coordination of work by PIC

iv) No proper communication established between PIC and operator.

3. Investigation Findings

 The operator did not use the soil provided for backfilling but instead excavated the soil
next to the ground beam to backfill.

Contribution Factors

 There was no proper communication established between PIC and Operator

 Utilities information was not provided to the PIC and Operator which resulted in the
operator hitting the existing Asbestos Cement (A/C) Water pipe (525 mm) that was 1

meter below the ground.

Root Cause

 The work was not supervised by a designated person.

 Banksman was not provided for the backfilling works.

 Information on nearby utilities not provided.

Conclusion

1. The PIC did not consider the importance of supervision as he failed to fully supervise the
backfilling works at Station 14 Entrance 2.

2. The operator took a shortcut and excavated the location where the existing Asbestos Cement
(A/C) Water Pipe were without having the complete information on the existing utilities layout at
site.

3. No clear communication between the Operator and PIC on the works to be carried out.

IMMEDIATE ACTION

1. WD stopped all ground beam backfilling works at Station 14 entrance 2.

2. WD cordoned off the affected area.
3. WD submitted Initial Incident Report
4. WD immediately notified the incident to AIS (Air Selangor)

6/1/2024 8.00 p.m.

Air Selangor contractor arrived at the scene, and further inspection was carried out. It was found that the water pipe
involved was 525 mm in size, and the damage was at the water pipe joint. The main valve for the damaged water pipe
joint was closed by Air Selangor, to prevent overflowing so that the appointed contractor from Air Selangor can proceed
with the patching works. The Patching work on the damaged water pipe was rectified at 5.00am on 7/1/2024. (Photo

It shall be the duty of every employer and self-employed person to ensure as far as practicable, the
safety, health, and welfare of all his workers.

5.1 Contractor’s General Safety Health Obligations

a) Responsibility: The WPC shall have full responsibility for the management and control of all safety
and health matters on the Site and may not delegate that responsibility not withstanding any approved
sub-contracting arrangements. The responsibility includes the safety and health of all persons working
on or visiting the Site including WPC of all levels, Other Works Contractors entitled to be on the Site and
the protection of the public in the vicinity of the Site who may be affected by the Works. The WPC shall
ensure that all workers employed are fully conversant with the safety and health requirements.

had obtained information that backfilling for ground beam works at Station 14 Entrance 2 work was ongoing,
the excavator used was WD's excavator.
