This document describes the investigation conducted for the Non-Occupational Incident (Death) of
Mr. MD Altap Hossain

An investigation team was established after the Post-Incident Review meeting to collect and analyze
documentation and to ensure the full repatriation and benefits of the deceased to the home country.

 Notification of Death to Family: AICC Site Manager contacted the family.

Brief Incident Overview


On 14 August 2023, at the Triple Bay Primary Infrastructure project sites i.e. Pump station 2,
Contractor (Al Ayuni) site clinic receives an emergency call regarding one worker complaining semi-
conscious and vomiting. Site emergency team attended the patient on site having high blood
pressure and vomiting.

Patient brought to Contractor site clinic and medical team contacted ISOS for assistance. ISOS team
attended the patient in site clinic and decided to take him to Central medical facility in Construction
village. The patient was diagnosed with hypertensive emergency, uncontrolled high blood pressure
235/110 with complication on the vital organs. Patient was stabilized onsite and referred to Al Wajh
Hospital as the activation of Helicopter was not approved due to weather conditions in Tabuk.

Patient was diagnosed with a stroke and was intubated and eventually went into a coma.

Patient was transferred to Tabuk, King Khalid Hospital. AICC was saddened to learn that the
employee had passed away on August 25.

AICC ensured that all the remains of the deceased were shifted to his home country and all the
financial compensation were made. Documented evidences were provided.

Immediate Actions Taken in Response to the Incident

 12:38 hrs. AICC male nurse received a call, from Road 02 SIPS 02, about a worker feeling
unconsciousness and vomiting.

 12:45 hrs. Male nurse arrived at the scene and attended the patient. Upon checking, it was
found out that his blood pressure was very high. AICC Male nurse brought the patient to
clinic.

 12:58 hrs. AICC male nurse called EOC and informed them about the patient and asked for
assistance.

 13:05 hrs. iSOS arrived and attended the patient and provided medical attention.

 13:28 hrs. iSOS left the AICC clinic and moved the patient to CMF for further medical
treatment.

AICC is tasked to perform the construction of infrastructure


of 25-30km² site on the coast with three pristine bays
Project that shall include the construction of the following:

- Roadways, earthworks, signage, road markings,


kerbs, etc.
- Street Lighting including associated low voltage
power networks
- Primary and Secondary Irrigation network including
associated pump stations
- Potable Water Distribution network including
associated pump stations
- MV & LV Power Distribution Cabling and
Substations
- Sewerage Gravity and Rising Mains including
associated pump stations
- Telecommunications/ICT Containment Network
- Storm water Drainage including swales, chutes,
channels, cut-off drainage, and
Embankment protection
- Wadi Crossings including associated culverts
- All temporary works required to complete the
construction works of the project.
- General site clearance after obtaining all relevant
approvals.
Specifically, the activities include:

- Installation of temporary facilities.


- Site preparation involving excavation, backfilling,
compaction and levelling;
- In-situ concrete works mostly in aprons;
- Construction of asphalt-paved access road which

will connect the site from the main highway;
- Slot drainage systems in aprons and underground
GRP pipes for storm water evacuation;
- Storm water cut-off channel;

In this instance, as there were no suspicious circumstances for Mr. Md. Altab Hossain's natural
death, a review meeting was held by Mr. Mohamed Farhan Abuseif to send his body to his home
country and get all his financial compensation from his employer “Rukan Al-Tameer Company
Limited (7009803623) “ .

The cause of death is recorded as brain death to ischemic stroke ‘Natural Death’ which was declared
following examination by King Fahad Specialist Hospital –Tabuk & Kingdom of Saudi Arabia Ministry
of Interior Civil Affairs Authority. This is a Non-Occupational related death.

medical treatment

ISOS Case Update:


14-August-23:
 Mr. Mohamed Altap Hussain (Patient) is 38-year-old Bangladeshi male with no know history of
hypertension brought to the site clinic with high blood pressure 235/110 associated with altered mental
status and severe vomiting.
 The patient was diagnosed with hypertensive emergency, uncontrolled high blood pressure with
complication on the vital organs.
 The patient was stabilized onsite and an attempt to activate the helicopter came negative due to
weather conditions.
 The patient was transferred to Al Wajah Hospital, discrepancy from MERP, as this is a life-threatening
condition, and an immediate therapeutic intervention is prompted.
 The patient was intubated on arrival and underwent CT brain which excludes brain bleeding. The
patient admitted to ICU under close monitoring.

15-August-23
 Patient remains admitted at AlWajh Hospital.
 Patient was intubated and ventilated as his conscious level had dropped further but an urgent CT scan
did not show any acute hemorrhage.

 Patient blood pressure improved on an infusion of blood pressure lowering medication, but he remains
on Ventilator.
 The plan of the hospital is to monitor him, repeat scan tomorrow and if unchanged stop the sedation to
and see if he can come off the Endotracheal tube tomorrow.

16-August-23:
 Patient in coma and remains on ventilator.
 Repeat CT today showed a posterior circulation stroke.
 Due to this his doctor at AlWajh has referred him to a higher center for more advanced monitoring and
care.
 Case update will be communicated when gets acceptance for the transfer via the Internal Saudi
escalation process.

17-August-23:
 Patient remains admitted to Al Wajh Hospital ICU in a critical condition.
 He is intubated and mechanically ventilated with severely depressed consciousness.
 The patient was diagnosed with a massive stroke to the brainstem and the prognosis is guarded.
 The request to transfer the patient to a higher facility came negative as the receiving advised that no
further treatment is indicated.
 The patient requires aggressive rehabilitation with close monitoring in a specialized center.

The site's first aid clinic didn't record any medical history/visits by the deceased employee during his
stay at the project.

 Investigation found there a basic medical examination is usually conducted for all new
workers at the project site to check the employee's health, but some of sub contractor's
employees did not receive this basic medical examination.

 AICC dedicated a senior-level Administration team to formalize the repatriation process at its
earliest to address the genuine grievances of the deceased family.

 Emergency response process initiated from the contractor through the EOC to ISOS was
found to be positive in reference to response times and care provided.
