HSE REPORT ON CROSS OVER SERVICE

BACKGROUND INFORMATION
On 31st of
December 2023, crossover service was held across various, Daystar Christian Centre
Locations as follows Oregun, Lekki, Ikorodu and Alimosho Centre. This report is a review of activities,
incident and hazards observed across various Centre and recommendations to help the church
mitigate against these risks in the future.

1. OREGUN CENTRE
The following were observed at Oregun Centre:
S/N Observation PR IM RI
1 As a result of the installed improvised A/C at the main auditorium, 3 5 15
there were trailing cables along the entrances 3 to 5 corridor that could
constitute trip and falls hazards. We had to redirect human traffic to
prevent incidents.
2 The additional A/C units in the main auditorium stopped working at 2 4 8
about 1930 Hour, resulting in serious discomfort to church members.
3 There was massive influx of congregation members at A3C and this 2 5 10
could be attributed to presence of Senior Pastor as a lot of member
wanted to catch a glimpse of the senior pastor. Even when the A3C
facility was filled and there were empty spaces at Annex 2A and 2B.
4. Partial lock down of the main gate into A3C was observed temporarily 4 5 20
and this constitutes a major risk in addition to the influx of members.
This was addressed observed through the intervention of Pastor Boye.
5 The fireworks boxes needed to have been surrounded using anti-blast 4 4 16
barriers to minimize impact of misfired fireworks.
6 Inadequate fire extinguishers to ensure fire risk is mitigated during the 4 4 16
launch of fireworks, as well as, in the event of fire outbreak around the
fully crowded facility.

2. Lekki Centre
The following were observed at Lekki Centre:
S/N Observation PR IM RI
1 An AC was leaking water causing a puddle at the back of the church. 1 3 3

Action Taken: The AC was switched off and Nehemiah mopped up the
water.
2 Bad toilets: 4 male toilets were out of service. 2 out of 4 of the adult 2 2 4
toilets were locked because they were not flushing. While the entire
children's male toilets were locked because there was a bad smell
coming from there.

Action Taken: The facility manager was notified, and they committed
to looking into it. The toilet at the changing room was also opened to
provide an additional toilet for the children's use. This may not have
been very effective since it was not announced.
3 Bad water: There was an issue with the pumping machine at the 2 2 4
facility which led to the water not being treated. The water is normally
pumped and treated before each service. However, the fault occurred
an hour before the crossover service. A plumber was called, and he
fixed the fault before service started. The treatment in the water could
not take effect before service began, so the water was very brown
during the crossover service. To my knowledge, this issue was
unexpected and was managed as best as it could be in the
circumstance.
4 The fire extinguishers in the facility expired in June 2023 and have not 4 4 16
been serviced till date, thus exposing the auditorium to fire risk.

3. Ikorodu Centre
The following were observed at Ikorodu Centre:
S/N Observation PR IM RI
1 The crossover Service went smoothly without any glitches or issues. 1 3 3
HSE unit and Protocol unit worked together to ensure hitch-free
operation. The gates were properly manned to ensure that children
were not allowed to go out of any of the gates unaccompanied by any
of their parents, no matter what they were going out to do. This really
help reduced the risk of child missing.

No Fire Works took place at Ikorodu Centre.

4. Alimosho Centre
The following were observed at Ikorodu Centre:
S/N Observation PR IM RI
1 Unsafe Act: 1(the Sparkles unit using brisco very close to the 2 5 10
Generator Sets)

Action Taken: They were told to stop and they did.


2 A sick teen was brought to the team for medical attention. 2 3 6

Action Taken: Attention of the Medical Team was drawn and they
attended to her.
3 The Unsafe Conditions were trailing cable. 3 2 6
4 The rug at the car park entrance not well taped to the floor which could 3 2 6
lead to trip and fall.
5 The light at the car park entrance door tangling and almost falling. 3 3 9

Action Taken: It was tapped to the wall of the entrance


Though no Fireworks took place at Alimosho Centre, operations went smoothly.

S/N Correction/Mitigation Actions Timeline Action Party


Oregun
1 The need to use slip-resistant floor coverings to cover trailing Procurement
cables at all design entrance and exit point should be discussed Manager
with vendors during contract initiation stage. This should be part
of the responsibility of the vendor to ensure safety of our
members,
2 The Central Cooling System needs to be repaired quickly to DCC Estate
eliminate the need for extra AC unit in the main auditorium
3 Need for Training, collaboration and simulation section on Q3 2024 Pastor Boye
crowd control, to ensure relevant unit are well informed on
modern crowd control measures.
4 Fire work vendors should be made to provide and make Procurement
available the following during fireworks launch: Manager
i) At least 2 fire extinguishers at location of Fireworks launch
ii) Anti-blast barriers to help minimize impact of misfired
fireworks.

This should be discussed during contract award and properly


monitored to ensure compliance.

Lekki
1 Discuss with Event Centre facility management on the need to Procurement
