Exact Location : Area 61 Storm water Manhole.

Description of
Near Miss :
It has been observed that on Monday 8th November, 2020 at area 61, a SBD foreman and
his crew were involved in a concrete pouring activity for the manhole construction for
storm water line with use of concrete bucket and mobile crane, while doing so concrete
got struck in the bucket and concrete was not coming from the bucket chute. Meanwhile
an employee unsafely climbed on top of the concrete bucket at the height aprox.2 meter
(working on the hanging load) without any fall protection / secure platform in place for
releasing the blocked concrete from the bucket.
Potential result
of Near Miss : Fall from height
Investigation carried out with involved crew advised concern foreman to use alternate
Immediate safe method to be used for release the blockage from concrete bucket /or change the
Actions taken : concrete bucket.

 TBT shall be conducted to Mr. Raji foreman and his crew regarding the near miss/safe working at
height procedure to avoid such violation in future.
