On 20-08-2020 around 10:35Hrs, one of our steel fixer Mr.Solaiman
Shabur Ali Gazi reported to SBD site first aid room with dizziness
and cramps on his lower extremities and abdomen. we had called
Programme Management emergency number by 10:40AM and
01 informed AD Port Control room by 10:41 AM. AD port ambulance
transported (10:55 AM) the above employee to Mafraq hospital for
further treatment. Illness person condition is stable and discharge
from Mafraq hospital on 20-08-2020 at 16:20 Hrs
3. Incident Findings shared online and cross verified by (SA)
Corrective action recommended by (SA)
SA- greeting the victim Solaiman (SS) and started investigation as
per investigation incident findings and answer by (SS)

Ms.Samar Yaseen (SA) – recommended some corrective action to
be taken to avoid such incidents
1. During high temperature standby AC bus should be
available near work location
2. Mr.Solaiman(SS) needs to complete general checkup and
report needs to submitted to AD Ports

3. TBT – Supervisors needs to check all workers health
condition, and anyone feel sick not allowed to work at site
4. SBD management should send a inter office memo to all
the SBD CMs, Site Engineers and Foremen if any workers
feeling unwell, they need to report to his activity
supervisor, and they have full rights to take rest.

implementation.
