On February 16th, 2023 time 15:00 PM while
BVIHSE Performed daily site inspection, found
unsafe condition angle grinding was not equipped
with a wheel guard and was also allowed to remain
connected to electric power, potentially causing
other workers to be injured when they accidentally
touch the angle grinding
