On July 15th, 2022 time 09:00 AM while BVI-HSE The workers and supervisor were given a safety

when working in the roof perimeter area and also installed
there was no lifeline installed in the area

condition there was no safety railing/guardrail
installed on the edge of the mezzanine staircase

Performed daily site inspection, found unsafe
action PIBK workers who were doing hot work
(welding) did not provide fire blankets to protect
sparks that fall down and there was also no lifeline
installed in the area

Performed daily site inspection, found unsafe
action PIBK workers did not wear special personal
protective equipment (full-body harness) when
working on scaffolding and also use scaffolding
that was not proper and also was not checked/
verified/tagged (green) by the certified scaffolding
inspector
