On December 14th, 2023 time 14:05 PM while BVI-
HSE Performed daily site inspection, found unsafe
condition unused material was not removed
immediately

Findings 2

Description: Description :
On December 14th, 2023 time 14:10 PM while BVI-
HSE Performed daily site inspection, found unsafe
action PIBK workers who were cutting the GRC
board used an angle grinder that was not equipped
with a wheel guard and also did not wear PPE
(face shields or safety goggles, masks, and
earplugs), potentially causing workers to be injured
when the cutting discs break

HSE Performed daily site inspection, found unsafe
condition the protruding rebar was not cut
immediately, potentially causing someone to
stumble

cutter knives did not wear PPE (Safety Gloves)

condition the power cable was left lying on the floor
and the power cable connection was not properly
protected, potentially causing workers to be
electrocuted
