1.0 HSE SITE INSPECTION

1.1 Content (activities)

Date: 12/10/2022
PIC: Wilfred Joming
 An HSE inspection can identify workplace hazards and serve as the catalyst for
the introduction of corrective action. In order to reduce risk and hazard, it is
necessary to identify potential areas of concern and communicate with PIC.
 For this month, we conducted HSE inspection along the site. We inspected the
item needed based on checklist of safety inspection form which included site
arrangement, personal protective equipment (PPE), working at height (WAH)
and others.
 So far, most of condition on site in good condition and there a few items that
need taken action for example maintaining the silt fence which is for temporary
barrier at construction sites for erosion control.

2.0 INSTALLATION OF SILT FENCE

2.1 Content (activities)

Date: 16/08/2022
 Construction often causes the soil on site and can contaminate nearby water
including river and lakes as stormwater runoff. Thus, the purpose of silt trap is to
retain the soil in disturbed land (for erosion control).
 Installation of silt trap also recommended for erosion and sediment control as
prescribed in [Akujanji 5.22(i)].
 We constructed silt trap near Kiansom Bridge (on the right) for this August.

3.0 HSE BRIEFING

3.1 Content (activities)

Date: 13/08/2022
PIC: Aldridge, Jais

 As previous month, our team (HSE team) have been conducted HSE Briefing on
th
13 August 2022.
 During HSE briefing, we talked about the usage of personal protective
equipment (PPE) and workers are advised to wear them at all times on site for
the purpose of preventing risks to their safety and health as prescribed in OSHA
1994.
 We highlighted the workers about machinery safety as they always deal with
them daily.
 These included operators are reminded to check their machineries such as

roller, excavator and lorry before and after work. It is to ensure that all
machinery is working properly and in good condition.
 The machinery should be turned off when not in use and placed in a safe
place.
 Other than that, we mentioned about housekeeping during working at height
(WAH). Workers are advised to practice proper housekeeping all the time
before, during and after working at height. It is to minimize any hazard and
accident happen.
 They also are reminded to make sure the platform is clear without abstraction to
avoid serious injury.

4.0 INSTALLATION OF SILT FENCE

4.1 Content (activities)

Date: 26/08/2022
PIC: Jahar
 Construction often causes the soil on site and can contaminate nearby water
including river and lakes as stormwater runoff. Thus, the purpose of silt fence is
to retain the soil in disturbed land (for erosion control).
 Installation of silt fence also recommended for erosion and sediment control as
prescribed in [Akujanji 5.22(i)].
 For this month (August), we monitored the installation of silt fences near
Tebobon 3. The length of silt fence near Tebobon 3 was 42 meters as shown in
Figure 4.1.
 We installed silt fence near Tebobon 3 because there is excavation and soil filling
work near there. The installation of silt fence also recommended by PIC, Jais for
erosion control.

5.0 OUTCOME (WHAT DID I LEARNED)

5.1 HSE Site Inspection

I learned how to inspect health, safety and environment on site. Through this
inspection, I can learn non-compliance condition on site and mitigation control required to
prevent these hazard and risk. Every work on site has hazard, so all workers should take
precaution to ensure that accidents and injuries do not occur.

5.2 Installation of Silt Trap

Silt trap at construction is important as it function in avoiding sediment flow into

stream and water. Silt trap is consisting of gabion stone that used as filter to prevent silt and
sediment flowing into flow. As I have learned, silt trap is one of effective method in erosion
and sediment control.

5.3 HSE Briefing

I was able to learn on how to communicate with workers and the importance of
safety and health to construction activities. As I learned in university, everyone is responsible
to maintain their health and safety to avoid any bad things happening at construction site.
There are many hazards and risks on site that should be emphasized by us (heath safety and
environment team) to ensure health and safety of our workers are guaranteed.

5.4 Installation of Silt Fence

Silt fence is one of method that used on construction site. Silt fence is to minimize
erosion and sediment control as I have learned at university. Silt fence is important on
construction site because it prevent as excess soil and silt from flowing into river or streams.
