Task 1: Accident investigation

Question 1 (a)
It is very important to secure the scene or place of the accident so that the evidence
cannot be removed or eliminated, to find out what material or scene caused or involved
the accident, and whether the accident was intentional or not.

The accident area is cordon of and install some prohibited access signs to preserved the
material and objects.

Question 1 (b)
The immediate actions taken to gather the information for the investigation area area:

1. Asking the trainee what happen on the accident and record it using the mobile phone
with the permissions.

Question 1 (c)
Communications that could have helped to prevent the accidents are
1. Safety Orientation/induction conducted by the WorkSpace Organizations to its two
contractors prior start of work and new workers on the project.
2. Conducting daily toolbox meeting every day before start of work.
3. Proper endorsement of work or area conditions every end of the shift initiated by the
Workspace to its two contractors.
4. Safety meeting to be attended by two contractor representatives so that all issues,
concerns on site or work are discussed and resolved.

5. Ensure all workers received appropriate health and safety trainings prior to start any
job.

Another answer:

The means of communications that could help to prevent the accident are the mandatory
safety induction for all new workers, conduct and participate to a daily toolbox meeting,
recognize and participate to any meeting and ensure all workers received appropriate
health and safety training prior to start any job.

Task 2: Health and Safety Culture

Question 2
Aside from insufficient communication, the indicators of a negative health and safety
culture in SC are:
1. SC did not attend or they mess the meeting initiated by the Workspace with invalid
reason or underestimating the job is small work.
2. The SC is reluctant to provide further health and safety information,
or the risk assessments and procedural statements (RAMS) which is required by

WorkSpace organization.
3. They provided old risk assessments (4 years ago) to the WorskSpace organization and
assumed that it is sufficient for their job.
4. The SC does not provide health and safety training to its workers especially in manual
handling even though it is only a small job and which the schedule trainings frequently
postponed.
5. The SC company does conduct safety training to its workers in the event of an
emergency incidents.

Task 3: Assessing the application of risk assessment

Question 3
The control measures that can be used to prevent or reduce are the severity of the
accident are:

1. The WorkSpace organization should select a contractor that has a good


implementation of Health and Safety Management System.

2. WorkSpace contractors site managers and workers must undergo safety orientation or
induction before working at the site or start on their job.

3. The SC contractor must have updated risk assessments associated with their
particular work routine and this must be communicated to the workers.

4. The Workspace organization and/or SC Site manager should initiate to conduct


inspections regularly in order to check the work area conditions if safe or unsafe before
the workers start as proactive way to reduce or eliminate the potential hazard.

5. The lead floor fitter should use other heavy materials instead of rollers to assist in
gluing the vinyl and place barricades and warnings in place to avoid passage of other
workers.

6. The accident can prevent if the SC workers should not take rush in unloading the
materials causing them to take shortcuts which they should unload in the unloading bay
and follow the instruction given to them to start their work 10:30 am on the next day
where the vinyl is possible stick already.

7. If the Workers has a manual handling techniques trainings and assess the access way
before transporting the materials.

8. They can use the mechanical equipment or transportation instead of manual handling.

9. Workers (experienced and the trainee) should wear head protection (hard hat with chin
strap) to minimize head injury in the event of any accident.

Task 4: Workers’ responsibilities in the workplace

Question 4
SC workers contravened Article 19 of C155 and recommendation 16 of R164. as
mentioned below:

(a). SC workers did not cooperate and fulfilled the instructions given by their site manager

(employer representatives).

(b). Workers do not have appropriate training in occupational safety and health especially
in proper manual handling techniques.

(e). The workers did not even ask the employer about all aspects of occupational safety
and health associated with their job as their right to work in safely manner.

(f). The workers is taking at risk at work with absence of their site manager during the
unloading of materials and where they also failed to report the accident.

Aside from written emergency procedure of Workspace (WS), the site manager should
add on his list the items below for the health and safety induction:
1) Health and Safety Policy
2) Permit to work system
3) Incident or accident reporting
4) Building and premises layout, including the assembly point.
5) List of emergency contact number / person
6) List of certified first aider
7) List of required documents (updated and approved risk assessment, method of
statement) prior to start the work.
8) The site or premises alarm system
9) Periodical or monthly inspection or colour coding.
10) Work timing / schedule.
11) Approved organization chart
12) The hazards and risks in the area.
13) Appropriate Personal Protective Equipment (PPE)

Task 7: Determining individual human factors that negatively influence


behaviour

Question 7
 The incompetent and unskilled young trainee from Smith’s Carpentry (SC) has
been allocated for the carpentry job in the storeroom and even requesting to help
to lift the wooden sheet even without safety induction and manual handling
training.
 The physical structure of young trainee is not yet fit to work for carpentry job and
manual lifting.
 The young trainee has no experience and knowledge about the task and even did
not receive any prior orientation and training from the experience worker or
supervisor.
 The attitude of experience worker is overconfident and complacent, has a poor

knowledge about the safety procedure, laugh about safety and even says “safety
is common sense”.
 The young trainee and experience workers were not aware about the risk
associated for the assign task, the risk assessment is outdated for the particular
job and proceed to perform the job without any control measures.
 The job performs without any supervision.
