Description of HSE Improvement Notice and how it was identified:

Despite issuance of Stop Work Notice (ARC-SWN-002, date: 08-02-2022) it is observed that Al Babtian allowed
the works (Use of scaffold) to be undertaken which is unacceptable as the transgressions noted on referred SWN
had not yet been completed and offered for approval. Also despite HINs (ARC-HIN-002 date: 02-12-2021, ARC-
HIN-016, date: 20-12-2021, ARC-HIN-033, date: 06-01-2022) and client HSE Manager instructions (email by Mr.
Roger Anderson dated: 04-01-2022), Al Babtain did not arrange necessary measures to ensure that all of the work
at heights and working platforms are safe for their manpower. Violating of MiSK City HSE Standards. Major Non-
Compliances and failures are following:

1. Scaffold with red tag is in use for height works.

2. Workers found working over Incomplete scaffolds and scaffolds with red tag, while no certifications submitted
for the scaffold erectors and inspectors to close the SWN.
3. Ladders are not being secured with proper angle and 5 rungs extending above the platform.
4. Incomplete scaffolds with missing guard rails, toe boards, access ladders, bracings.
5. Scaffold erection over soft ground without sole boards underneath.
6. Scaffold erectors are not 3rd party certified, either scaffold erected by Labor only.
7. Scaffold inspector is not 3rd party certified.

sole board provided for the scaffold. tagged and must dismantle.

Corrective Actions Required:

Al Babtain Project Manager instructed to comply the requirements of Stop Work Notice (ARC-SWN-002, date: 08-
02-2022) and HINs (ARC-HIN-002 date: 02-12-2021, ARC-HIN-016, date: 20-12-2021, ARC-HIN-033, date: 06-01-
2022).
- All scaffold erections and inspections by the competent personnel only. Submit 3 rd party certificates for the
scaffold erectors and inspectors.
- Provide approved RAMS for scaffolding works.
- Permits to be issued with approved RAMS, approved drawings and othere relevant approved documents
in place.
- Ladders must be secured with proper angle and 5 steps above the platform.
- Scaffold over soft ground must have sole board underneath.
- Scaffolds must be inspected by the scaffold inspector within the inspection time frame.
