1. What financial arguments could you use to justify your proposed recommendations to
segregate FLTs and the workers?

As per the scenario, Due to new FLT drivers, workers and properties get damaged. It’s lead to
financial issues. they are Direct and In direct cost.

Direct cost:

 Fines & Penalties – As per the legislation accidents are lead to criminal court.
 First aid or medical cost – If workers get injured the medical cost belongs to
company.
 Sick pay – As per scenario the workers is expected to be off work for six weeks to
recover from the injury.
 Compensation – The injured worker seeking legal advice in order to claim foe
compensation.
 High insurance premium – Due to accidence company premium get increased.
 Over time is increased – Due to absents over time needed.

Indirect cost:

 Loss of staff morale in the organization – Workers loss faith in management.


 Damage to public image and business reputation – Reduces in customers.
 Cost of recruiting and training temporary or replacement of labour – Difficulties in
workers adaptation.
 Cost of remedial action following an investigation.
 General difficulties in recruiting and retaining staff.
 Compliance with any enforcement notice served.

2. You email a report to the overall Store and Warehouse Manager.

(a) what unsafe behaviour would be included in the report?

The following points are based on scenario,

 Lack of health and safety knowledge leads to “Rules braking” in warehouse.


 The goods and products are blocking the walkways. So workers get diverted to
vehicle routes.
 There is no written records followed by any near misses between FLT and workers,
 Poor housekeeping – oil spillages and damage.
 Unskilled drivers – leads to collision and property damage.
 Poor hospital facilities – 8km from the store.
 No health and safety representative.

(b) what voluntary feedback would be included in the report?

According to scenario, The following worker feed backs are.

 Poor working environment.


 FLT skipped and goods damage
 Poor management support.
 Using mobile while working hours,
 Lack of training.

3. When selecting the external contractors, what would you consider when assessing
their competence?

The Basic requirements for the selection of contractors are listed below , as per the scenario

 Company need Health and Safety policy – because no health and safety followed in
the workplace.
 Their past risk assessment reports – Risk assessment is main source for safety.
 Qualifications and training records – To eliminate the unskilled workers.
 Maintenance of equipment – For good working condition.
 Experience of similar works – For 100% of involvement because, some workers get
distraction here.
 Membership of a professional organization – lack of professional observed.
 Previous or current clients details – To ensure reputation.
 Accidents record – To verify the accident rate. Because, we already facing accident
issue.
 Enforcement action – To view their action towards the work,
 Adequate resources – for lack of resource.
 Details of Quality assurance and quality control practices currently in place for the
execution of similar work.
 Copies of ISO 9001, 14001, OHSAS 18001 or any other accreditation and
certifications as applicable.
4. Based on the scenario only, what are the likely benefits to the organisation of having
this formal safety management system?

ISO 45001 is an International standard that specifies requirements for an occupational health
and safety (OSH) management system.

 Improve employee safety – The store has many accident records. So, we follow this
point.
 Boost stakeholder confidence – Due to the accidents stakeholder lost trust. So
maintain zero accident.
 Increase sales or productivity – By proper maintenance of warehouse.
 Lower insurance costs – Eliminate accidents and secure workers lives.
 Foster management involvement & accountability – Here, we need management
involvement.
 Reduce risk – Need safety monitor.
 Identify opportunities – Need of external contractor.
 Stand out as an industry leader – Support for workers and coordinate.
 Gain a competitive advantage – By getting Knowledge staff.
 Engage employees in safety – Need their involvement.
 Shift from reactive to proactive – Split the shift timing.
 Demonstrate your commitment to safety – Create safety oriented training.

5. What are the negative indicators of safety culture at the warehouse?

 Poor health and safety is followed by the management.


 Rules braking culture followed in warehouse.
 Many obstacles blocking the walkways poor housekeeping observed.
 Increased injuries and accidents – It is the most significant consequences of poor
safety culture. Here FLT and worker get collision.
 High cost – Due to the accidents workers get injured and they looking for
compensation.
 Damaged product – Due to oil spillage, FLT get skipped and collide with properties.
 Damaged reputation – According to the accident report store reputation get damaged.
 Legal issue – Store facing many legal problem in case of many accidents.
 Increase in worker turnover – Many absenteeism occurs. So, overtime provides to
complete work.
 Decrease in sales – company name get damaged and loss of customer.
 Lace of focus in drives – They using mobile during work,
 Poor reporting and accident records – there is no records of accident in management.

6. What do you need to consider so that first-aid needs are realistic and proportionate
for the warehouse workers?

The First aid factor consider for the warehouse workers are,

 The general risk level of the workplace – The warehouse risks are Walkways
blocking, Workers step into vehicle route, Property damage and oil spillage.
 The hazards present in the workplace – The main hazard is oil spillage.
 Accident history – Here, one person get fracture or leg broken’
 Vulnerable persons – Here, there is no persons likely.
 The number of workers – Due to accidents there is more absenteeism. Leads more
worker.
 The distribution of workers - There more in one area or at a certain time of day.
 Work patterns and shift systems(adequate cover must be provided for all shifts,
including weekend and nightshift operations.)
 Workplace location (geographic) – schedule the workers location on time.
 The spread of the workplace – control group forming.
 The work pattern of staff – provide pattern of work.

7. Decide what level of investigation (minimal, low, medium, high) is appropriate in this
case, clearly justifying each step you take that leads to your decision.

HSG245 states that “an effective investigation requires a methodical structured approach to
information gathering, collation and analysis” and as such, suggests a four-step approach.
The four steps featured in the guidance are as follows.

1. The gathering of information.


2. The analysis of information.
3. Identification of risk control measures.
4. The action plan and implementation.

Step1 – Gathering of information


 Secure the scene of accident on the warehouse.
 Identify the witnesses in place of incident.
 Collect factual information like photo, measurements, notes, mark up plans and
sample in place of warehouse.
 Interview witnesses about warehouse accident.
 Examine documents of site plans, safety policy, risk assessment, Training records,
safety system of work, Maintenance record and sickness record.

Step2 – The analysis of information

 Immediate causes
o Unsafe acts – Here incompetent, young FLT driver was assigned and he using
mobile phone while working.
o Unsafe conditions – Here hospital is 5miles fare from the store.
 Underlying or Root causes
o Reason behind the immediate cause.
o Often failure in the management system like no supervision, no training and
no risk assessments.

Step3 – Identification of risk control measures

For Immediate causes

 Clean up the oil spillages.


 Replace the blocking obstacle

For underlying or Root cause

 Provide training to young workers.


 Maintain good housekeeping.
 Need to make changes in management system.

Step4 – The action plan and it’s implementation

 Dangerous conditions must be dealt with immediately – oil spillage.


 Interim action may be possible.
 Underlying causes will require more complex actions – will take time, effort, money
and need for prioritisation.

8. What individual human factors could have negatively influenced the behaviour of the
young FLT driver?

The main human factors that negatively influences the behaviour of the young FLT driver are
following,
 Safety culture is not followed in the warehouse. This may leads to bad safety
behaviour in the workplace.
 There is no proper induction training provided. So rules breaking behaviour formed.
 Due to poor housekeeping, the goods were blocking designated walkways. So, it
influences bad safety behaviour.
 Poor reporting of hazards, like blockage of walkway. It’s influence workers step into
the FLT vehicle route.
 There is no proper policies implemented in management. So, there are many health
and safety issues in warehouse’
 Incompetent driver appointed – there is young drivers were driving FLT.
 Lack of supervision. It leads to young driver using mobile phone while driving. And it
cause FLT collision with workers.
 There is no proper records of accidents and near miss – it leads to get accident and
person get leg injury asking for sick pay.
 Poor inspection – cause the management rarely visit warehouse. Bad safety behaviour
followed,
 There is no safety representative in management – it may cause enforcement act
towards the store,
 Bad Knowledge about legislation – it increases the company premium.

9. Based on the scenario only, what management failures could have contributed to this
accident?

 Health and safety ignored in store – it is the main root of accident, if store follows the
health and safety properly no accidents occurs.
 Store fails to appoint a responsible persons – it cause bad safety culture in warehouse.
 Failures to carry out risk assessment – There is no proper records followed in store
and near miss and accidents are not recorded
 Bad induction training for the young drivers and they don’t follow the rules and using
mobile phone while driving.
 Lack of supervising and leadership – there is responsible person to guide the workers
about health and safety.
 No feedback system followed – if they get feedback from the worker that accidents
are prevented.
 No policies were displayed in the warehouse – It lead to change worker mind as
safety is not important.
 Never follow Tool box talk – Every before start work and clarify the risk and hazards
in the warehouse.
 Failures to provide safe system of work or Standard operating procedure (SOP) – to
begin or how to work safely.
 Fails to conduct safety inspection – for every year conduct safety inspection to ensure
health and safety in warehouse.
 Lack of awarding system – to motivate workers in health and safety award them for
every safe works.

 Segregate the obstacles in FLT route and walkways.
