Description of HSE Improvement Notice and how it was identified:

Based upon site visit on 07-02-2022 and 08-02-2022 and despite being raised in SOR’s Al Babtain did not
arrange necessary control measures to ensure that all of their excavations are protected which is unacceptable.
Violating the MiSK City HSE Standards. Major Non Compliances and failures are following:

1. Excavating the trenches without barriers in place.


2. Leaving trenches open in fromt of the gates for residential buildings.
3. No safe access arrangements for the workers.
4. Excavation hazard signs not installed.
5. Missing caution lights for the trenches in public areas.
6. Cable mismanagement creating tripping hazard for residential access ways.
7. Materials being placed at the edge of excavation.
8. Improper electrical joints for the strip lights.

Al Babtain site management instructed to comply the requirements as:


- Ensure all excavations are fully protected with barriers.
- Edges for the trenches must be fulyy barricaded including front of residentials must be properly secured.
- Safe access must be provided for the site operatives.
- Install excavation hazard signs.
- Install caution lights.
- No materials to be placed at the open edges of the excavation.
- Cable management need improvement to avoid tripping hazard.
- Avoid jointing in the cable. provide proper electrical joints.
- A report by the Electrical Engineer confirming that temporary electrical installations are safe for any
electric shock / electrocution as public areas are critical because of public interface.
