1- NON-COMPLIANCE TO ISG-OP430 GENERAL

HEALTH & SAFETY, THE WORKER IS NOT
WEARING THE REQUIRED PPE (GLOVES, SAFETY

GOGGLES) (PPE)

2- NO BARRICADING (UNSAFE CONDITION)

3- POOR REACH (ERGONOMIC) WHICH CAN CAUSE
MUSCULOSKELETAL DISORDERS. (UNSAFE ACT)

4- NO WORK PERMIT. (NON-COMPLIANCE)

RECOMMENDED CORRECTIVE ACTIONS:

1- Provide instruction and required PPE to the whole team to follow safety of ISG by means of toolbox talk (attach a photo
and signed attendance as proof of action).
2- Work area must be barricaded to warn and alert others of the hazards created by the activity being done.
3- Worker supposed to use a ladder to avoid any excessive reach.
4- Inform HSE department to obtain a work permit to identify necessary measures prior to starting the work.
