PREFACE
Director (Refinery) of erstwhile Cochin Refineries Limited (CRL) appointed a committee in 1994 for
devising a procedure of standard operations. The committee formulated a system named Refinery
Operating System (ROS). A set of ROS was formulated and released by the committee. Over years,
the ROS was evolved to a well-documented Refinery Operating System (ROS), which outlines the
Management System Procedure for various business processes in the refinery. There were 90
approved ROS and 12 ROS under formulation before the current edition. Prior to 2015 edition,
Project (Technical) department, which was earlier known as Corporate Planning, before
organizational re-structuring in 2006, was the custodian of the ROS.
In 2014, General Manager (Operations) constituted a task force for a thorough revision of ROS in
terms of the purpose, relevance, scope and work flow in the current business context vide office
note KDD/03/09 dated 28-02-2014.
The use of radiation sources of various types in industry is widespread and is increasing steadily all
over the world. Open field industrial radiography for Non Destructive Testing of weld joints is the
very common application of radio isotopes in our refinery. ROS-023 - Regulations of Handling and
Use of Radiographic Devices has been developed for better regulatory control of open filed industrial
radiography.
Use of radio isotopes for nucleonic gauges was not common in our refinery until the commissioning
of Continuous Catalyst Regeneration Reforming unit. In this unit, eight Ionizing Radiation Gauging
Devices / Nucleonic Gauges are available. It is understood that more such devices are envisaged in
the refinery along with new project Integrated Refinery Expansion Project (IREP). As the regulatory
requirements of Ionizing Radiation Gauging Devices / Nucleonic Gauges are different from of open
field industrial radiography, it was decided to formulate a separate ROS for Ionizing Radiation
Gauging Devices / Nucleonic Gauges.
This ROS outlines the procedure/ regulatory requirements for procurement, storage, installation,
operation, decommissioning / disposal of Ionizing Radiation Gauging Devices / Nucleonic Gauges in
BPCL Kochi Refinery.
The Task Force was constituted by Mr. Soman C. K., General Manager (Operations) in 2014 for
revision of the existing Refinery Operating System.

review and finalisation of this document.
1 INTRODUCTION
Industrial application of radiation sources is increasing steadily all over the world and various
sources of ionisation radiation are widely used for this. Nucleonic measurement and control
system, popularly known as nucleonic gauge, is one such application and has found significant
and indispensable usage for measurement and control of parameters such as thickness,
density, level, composition etc. While using ionisation radiations and radioisotopes, it must be
borne in mind that they should be handled and used in a proper and safe manner. Otherwise,
they may be harmful to the workers and public.
1.1 PURPOSE
The purpose of this ROS is to ensure the safe usage of Ionizing Radiation Gauging Devices
(IRGD), Nucleonic Gauges (NG) and XRF machines at BPCL Kochi Refinery and to comply with
requirements of Atomic Energy Regulatory Board (AERB) and Atomic Energy (Radiation
Protection) Rules, 2004.
1.2 SCOPE
The scope of this Refinery Operating System (ROS) is to establish guidelines / procedure to be
followed for procurement, installation, operation, decommissioning and disposal of Industrial
Ionizing Radiation Gauging Devices (IRGD) / Nucleonic Gauges (NG) and XRF machines at BPCL
Kochi Refinery.
The scope of this ROS includes all nucleonic gauges used in BPCL Kochi Refinery and are
applicable to all nucleonic gauges satisfying the following (but not limited to)
i. using ionizing radiations namely Gamma, X-ray, Alpha, Beta and other particles that
can induce ionization. E.g. Nucleonic gauges level measurement used in CCR Unit.
ii. XRF machines used for Positive Material Identification (PMI), used as analysers E.g.
Sulphur analyser used in QC. In case of XRF machines registration with AERB is only
required and hence clauses related to registration are only applicable to them.
1.3 COVERAGE
This ROS is applicable to Kochi Refinery and its connected facilities under its administrative
control as listed below
1. Kochi Refinery
2. Jetty Facilities like NTB, STB, & COT (when job is directly executed by KR)
3. Pipeline corridor
5. SWSS Aluva
1.4 LIMITATIONS
Requirements of ROS shall not conflict with Legal and other requirements published by the
Govt. of India / AERB from time to time. However, if the requirements of this ROS are more
stringent than the requirements of the above, then the requirements of this ROS shall govern.
1.5 EXCLUSIONS
1. Regulations of open filed industrial radiography devices are not covered in this ROS and are
covered in ROS-023.
2. Operational emergencies arising out of failure or malfunctioning of IRGDs / NGs or
connected instruments are not covered in this ROS and the same may be covered in
operating manual or standard operating procedure.
1.6 DEVIATIONS
The procedures / practices described in the ROS are intended for standardization of the
processes involved with installation and usage of ionizing radiation gauging devices /
nucleonic gauges, and hence the same shall be strictly adhered to. However, deviation, if any,
arise due to unavoidable circumstance(s), shall be approved by Head of Kochi Refinery. The
deviation note in the prescribed format specified in ROS-001 Appendix ‘ROS-001/A9.4: Format
for Approval of Deviation in ROS’ shall be initiated by the HOD of the department proposing
the deviation through the Functional Head.
This section lists all definitions and abbreviations used for the purpose of this ROS.
2.1 DEFINTIONS

3 REQUIREMENTS
The following procedures shall be followed for the procurement, storage, installation,
operation, decommissioning and disposal of IRGDs / NGs.
The nucleonic gauges also known as ionising radiation gauging device (IRGD) contain sealed
radioactive source(s), radiation shields, useful beam controls and other components, which
form an integral part of the device. The nucleonic gauges are used for the purpose of
determining and / or controlling thickness, density, moisture, level, interface location, and / or
qualitative or quantitative chemical composition. These IRGDs are being used by various types
of industries. These devices are provided with maximum built-in safety features and
consequently need minimum operational controls. Different types of radiation sources with
activity ranging from several MBq (mega Becquerel) to hundreds of GBq (Giga Becquerel) are
being used for this purpose depending on the applications and characteristics of sources. X-ray
sources are also used in place of sealed radioactive sources.
1. Depending upon the use, nucleonic gauges are classified as “Static Gauges” or “Continuous
Gauges”. Static gauges are either “Portable gauges” or “Fixed gauges” and are used to
make measurement at a time on the individual sample. Continuous gauges are used to
monitor the product on-line.
2. IRGDs / NGs are used in variety of applications, such as non-destructive testing, level
indication system, thickness gauges, density gauges, material composition detectors and
analytical X-ray equipment.
3.2.1 PROCUREMENT
1. Only devices of type duly approved by AERB shall be procured and used. If the type of the
device is not type approved, the same shall be obtained before procurement by
submitting necessary documents to AERB detailing demonstrating compliance to relevant
national / international standard.
3. In case of import of IRGDs / NGs containing sealed sources, a ‘No Objection Certificate
(NOC) for Import’ as part of procurement shall be obtained from AERB. The Engineer in
Charge intended to import the IRGDs / NGs shall apply for NOC for import in the
prescribed format (Form ID. AERB/RSD/NG/PROC).
4. In case of IRGDs / NGs being procured by the LSTK package contractor / PMCs for major
projects, the authorisation / no objection certificate for procurement and / or import
shall be obtained by the respective LSTK contractor / PMC in the name of BPCL Kochi
Refinery. The Engineer in Charge of the project shall ensure this and obtain the copy of
the documents before the IRGD / NGs are brought to BPCL Kochi Refinery. Upon arrival of
the material at BPCL Kochi Refinery, the original documents shall be obtained by the
Engineer in Charge and shall be forwarded to the HOD of Fire & Safety department.
1. Immediately upon the receipt of IRGD / NG consignment at BPCL Kochi Refinery, a joint
inspection by Radiological Safety Officer (RSO) and Engineer in Charge, who intended the
IRGD / NG, shall be conducted. The following shall be ensured during the joint inspection.
a. The radiation level shall be measured and confirm readings are within the acceptable
range.
b. Confirm that the gauges are type-approved and as per authorisation by AERB.
2. If any discrepancies noticed in the received gauges from the authorized model, activity and
the source, the engineer who intended the IRGD / NG shall take up the matter with vendor
for necessary amendment in the AERB authorization or replacement.
3. If the received gauges are intact and are as per the AERB authorization, the Engineer in
Charge who intended the IRGD / NG shall intimate the receipt of nucleonic gauges to HOD
of Fire & Safety department with details. The HOD of Fire & Safety department shall
intimate the receipt of IRGD / NG to AERB in the prescribed format
AERB/RSD/NG/PROFORMA (A), refer Appendix ROS-024/A9.2).
4. In case of IRGD / NGs procured through the LSTK contractor, the Engineer in Charge of the
job shall coordinate with the contractor and intimate the receipt of the gauges at KR
premises to HOD of Fire & Safety department. The HOD of Fire & Safety department shall
intimate the receipt of IRGD / NG to AERB in the prescribed format
AERB/RSD/NG/PROFORMA (A).
5. Nucleonic devices pending installation shall be stored securely in an exclusive room under
lock and key with caution placard by the indenting department / section for security and
safety reasons. The RSO shall conduct a monthly inspection of the storage room and
ensure the safety and security of the device.
1. The Engineer in Charge who intended IRGDs / NGs shall ensure that the devices are
installed and commissioned by the supplier of the devices or their authorised agents
thereof.
2. During use of the devices, the HOD of the owner department of the facility shall ensure
that there is no permanent occupancy in the immediate vicinity (i.e. within a distance of 3
m) of the installation.
3. HOD of the owner department of the facility where IRGD / NG is installed shall ensure that
an SOP for Operation and Maintenance of the IRGD / NG is available in the Dept / Section.
The SOP shall warrant prior permission from RSO of the refinery for working in the
immediate vicinity (i.e. within a distance of 3m). Source shield house shutters of all IRGDs /
NGs shall be closed and locked during turnaround or shutdown period.
4. A caution placard shall be installed near the installation of the device meeting the
specification of AERB.
5. The Engineer in Charge of the intending department shall intimate installation and
commissioning of IRGD / NG to HOD of Fire & Safety department within 7 working days.
6. Upon the receipt of the intimation from Engineer in Charge, the HOD of Fire & Safety shall
intimate the installation and commissioning to AERB in the prescribed format
(AERB/RSD/NG/PROFORMA (B), refer Appendix ROS-024/A9.3) within 7 working days. He
shall also forward the application for registration of nucleonic gauges in the prescribed
format (AERB/RSD/NG/REG, refer Appendix ROS-024/A9.4). AERB is in the process of
extending the registration of IRGD / NGs through their online portal e-LORA. Once the e-
LORA is enabled with the registration of the IRGD / NG, RSO shall initiate the registration
through the portal with the consent of HOD Fire & Safety.
7. After installation and commissioning, the Engineer in Charge of the intending department
shall organize a meeting with RSO / Representative from Fire & Safety, other concerned
departments / sections and the vendor for handing over all related documents / details
and for ensuring the compliance of AERB guidelines in installation and commissioning.
3.2.4 DOCUMENTATION
The Engineer in Charge of the indenting department / section shall co-ordinate with vendor
for the submission of following documents to Fire & Safety department / RSO.
1. Copy of Type approval or exemption letter for the nucleonic device from AERB.
2. Copy of an undertaking, which was submitted along with application for type approval
from manufacturer / vendor for safe disposal of the radioactive sources incorporated in the
nucleonic gauge.
3. Authorization / No objection certificate from AERB for the procurement and use of
nucleonic devices.
4. The manufacturer / vendor shall provide the test certificate copies related to safety
performance classification given to the nucleonic device as per AERB.
3.3 OPERATION
The following section details the requirements for the operation, servicing / maintenance
activities and reporting of the status.
RSO shall conduct radiation level monitoring once in a month at the external surface of the
devices, at one metre (1m) from the device and at the nearest occupied locations. The reading
shall be recorded in a register kept with RSO.
RSO shall ensure that the device is serviced and maintained ONLY by the supplier of the device
or their authorized representatives.
When open field radiography is carried out in the proximity of the location of IRGDs / NGs,
functioning of IRGDs / NGs may be affected due to interference of the radiation from the
source of the radiography devices. Appropriate measures like temporary shielding of the
IRGDs / NGs at the time of radiography should be taken and work permit for carrying out such
radiography shall be countersigned by owner department of the facility.
HOD of Fire and Safety department shall sent a status report of all IRGDs / NGs under the
possession of Kochi Refinery in the prescribed form RSD/NG-STATUS/PROFORMA (C), refer
Appendix ROS-024/A9.5) to AERB twice in a year; i.e. in the month of January and July in every
year.
1. All IRGDs / NGs, which have completed its useful life / are not intended for further use
shall be decommissioned and disposed off in consultation with AERB.
2. The Section Head of the user department of IRGD / NG shall give intimation to HOD of
Fire & Safety department about the requirement of decommissioning and disposal of
IRGD / NG on the expiry of their useful life.
3. Based on the request from end user, HOD of Fire & Safety department shall forward the
application to AERB seeking permission for disposal and transportation of IRGD / NG.
4. After obtaining permission disposal and transportation from AERB, the IRGD / NG shall
either be returned to the original manufacture in the country of origin as per the original
undertaking or be sent to BARC / approved nuclear waste disposal facility of the
Department of Atomic Energy, Government of India for disposal. Document regarding
disposal of the device shall be obtained from the original manufacturer, BARC or
Department of Atomic Energy as the case may be.
5. Nucleonic devices that are removed from the installation and pending for disposal shall
be stored securely in an exclusive room under lock and key with caution placard by the
Fire and Safety for security and safety reasons. The RSO shall conduct a monthly
inspection of the storage room and ensure the safety and security of the device.
6. Existing procedure for asset disposal as per Asset Disposal Authority (ADA) shall be
followed.
1. The HOD of Fire & Safety department shall ensure that at least one person is available in
Safety Section as Radiological Safety Officer (RSO) who is trained and successfully
completed an approved training course on radiation safety aspects including IRGDs / NGs.
2. The SBU Head shall nominate one of the persons who have successfully completed the
approved training course on radiation safety as Radiological Safety Officer.
3. The nomination of the Radiological Safety Officer shall be applied through portal of AERB
e-LORA (e-Licensing of Radiation Applications) for their approval.
1. HOD of Fire & Safety department shall ensure that a minimum of two radiation survey
meters are available in the refinery and are calibrated and maintained at all point of time.
The record of all radiation survey meters available in the refinery shall be maintained at
Fire & Safety department.
2. RSO shall ensure that all radiation survey meters available in the refinery are calibrated
once in a year and calibration certificates are available. Copy of the calibration certificates
of all radiation survey meters shall be available at Safety Section of Fire & Safety
department. The radiation survey meter shall be locally tested once in 6 months using a
check source for ensuring function of the meter. If error is observed to be more than 20
%, the survey meter shall be sent by the user to the supplier for calibration and
correction. Section Head of Maintenance-Instrumentation shall coordinate the repair and
calibration of all radiation survey meters available in the refinery.
1. Even though the nucleonic gauges are designed and manufactured with maximum built in
safety features, untoward incidents involving IRGD / NG may lead to radiation
emergencies. The occurrence of any one or more of the following situations may be
deemed constitute a radiation emergency:
e. Leaking of source.
2. HOD of Fire & Safety shall ensure an Emergency Response Plan with respect to each of the
above radiation emergencies and shall be in line with the model emergency plan published
by the Radiological Safety Division of AERB. The same shall be reviewed periodically once in
three years.
3. The Emergency Response Plan shall be rehearsed once in a year by conducting mock drills
on any of the emergency scenario mentioned in the plan. The functioning of all personnel
in the emergency plans shall be tested. A review meeting shall be conducted after the
exercise. The lessons learnt from the exercise should be judiciously incorporated in the
emergency response plan.
The owner of the facility where IRGD / NG is installed shall ensure that
1. Daily monitoring of the physical condition of the device is conducted and records
maintained in the log book. Any abnormality noticed shall be intimated to RSO / HOD of
Fire & Safety immediately.
2. No work permit shall be issued for working in the immediate vicinity (i.e. within a distance
of 3m) from the device, without obtaining clearance from RSO / HOD of Fire & Safety
department.
3. No permit shall be issued for dismantling, modifications and maintenance work of IRGDs
without obtaining written clearance from RSO.
4. Ensure that no permanent occupants are available in the immediate vicinity (i.e., within a
distance of 3m) of the device.
5. No visitor / outside agency shall be allowed to the IRGD location without accompanying
personnel from KR.
6. The radiation survey meter available in the Department / Section are calibrated and in
working condition. Radiation survey meter shall be handed over to Instrument
maintenance section for re-calibration once in a year and rectification job, if not working.
7. The employees in the facility where IRGD / NG are installed are aware of their roles and
responsibilities during a radiation emergency as mentioned in the radiation emergency
response plan.
1. Ensure that all original records related to the regulatory requirement to own and use
IRGDs at Kochi Refinery, including disposal records, if any, are available. Any
discrepancies noticed shall be reported to the MHSSEC.
2. Liaison with AERB and forward all applications with regard to the IRGD / NG
procurement, receipt, installation, commissioning, transport, decommissioning and
disposal.
4. Prepare a Radiation Emergency Response Plan and conduct mock drills thereof once in
a year.
5. Communicate to AERB about the emergency occurrences, if any, within 24hrs and
submit a detailed report on the incident after carrying out investigations.
6. Filing written complaint with the local police station, in case of loss or theft of
radioactive source, if it is not traceable within 24hrs.
8. Review of the ROS with respect to AERB regulations and initiate revision of the ROS, if
necessary.
9. Communicate all relevant changes in the AERB regulations to all concerned in Kochi
Refinery.
10. Ensure at least one RSO who is experienced and successfully completed an approved
training course on radiation safety aspects including IRGD / NGs is available in Safety
section and provide periodic refresher training to RSO on radiation safety.
1. Conduct radiation level monitoring within the vicinity of IRGDs / NGs once in a month
and maintain record of same.
2. Ensure that all radiation survey meter available in KR are calibrated once in a year and
the function tests are conducted in every six months.
3. Conduct a monthly inspection of the storage room and ensure the safety and security of
the devices in the storage rooms.
4. Investigate any unusual situation that could lead to potential exposures and initiate
remedial measures.
5. Adhere and ensure compliance of the terms and conditions of the license issued by
AERB.
8. Applying the registration of IRGD / NGs once the e-LORA is enabled with the
registration of the same. Refer 3.2.3(6) of this document.
5 APPROVAL
This ROS undergoes a review and approval process in line with the process specified in Section
5.0 of ROS-001.
2. Reviewing authority shall ensure that requirements of ROS are relevant with respect to
context of the business and are in line with current environment.
3. Reviewing the authority shall sign on the Document Release Note of this ROS.
1. Refinery Council (RC) is the approving authority for the approval of this ROS. Head of KR is
the signatory on behalf of the RC and shall sign on the Document Release Note of this ROS.
2. AERB periodic inspection is another monitoring and control measure. Fire and Safety shall
coordinate the AERB inspection and ensure the compliance of the recommendation by
the AERB.
7 RECORDS
1. Application for authorisation / no objection certificate for use of IRGD / NG available at
KR.
6. Copy of the half yearly status report sent to AERB on all IRGDs / NGs
