Employee Request for Salary/Cash Advance

Repayment Terms:

next scheduled paycheck

I authorize the deduction of the remaining balance from my final paycheck should my
employment end prior to full repayment of the salary/cash advance.

In the event of my resignation, termination or any other cause for discontinuation of my

employment, the remaining balance of my salary/cash advance shall be immediately
due and deductible from my last paycheck.
