AFFIDAVIT OF DISCREPANCY

I, MARLYN G. CORPUZ, of Legal age, married, Filipino Citizen ,and a

resident of Blk 48 Lot 17 Phase 1-A, Southville Brgy. Inocencio Trece Martires City,
Cavite, after having been duly sworn to in accordance with law do hereby depose and
state:

That when I got the copy of my Live Birth Certificate from National Statistic
Office (NSO) my name entered and appeared as MERLYN S. GAOIRAN.

That I used MARLYN S. GAOIRAN in all my legal documents like Voter’s ID,
Marriage Contract and other legal ID’s.

That the name MARLYN S. GAOIRAN which I used in all my documents and
MERLYN S. GAOIRAN appeared in my Birth Certificate from National Statistic Office
(NSO).

That the name MARLYN S. GAOIRAN and MERLYN S. GAOIRAN refers to

one and the same person.

That I am requesting the concerned authorities to amend /change my name from

MARLYN S. GAOIRAN to MERLYN S. GAOIRAN;

That I am executing this statement to attest to all the above facts, and to whatever
legal purpose it may serve.
