PARENTAL CONSENT TO USE STUDENT DATA

In accordance with the Department of Education’s mandate to protect and promote the
right to and access to quality basic education, DepEd collects various data and information,
including personal information, from various subjects using different systems, online or offline.

In this regard, the undersigned parent VOLUNTARILY: (a) consents that the school will
process the various data and information, including personal information of my child; (b) allows
the posting of data and information, including personal information of my child in the official FB
Page of the school and the FB Groups that will be formed; (c) agrees that pictures and/or
videos where my child is included will utilized by the school for educational purpose and allows
that it will be posted in the official social media accounts of the school.
