Reason: No module up to present and due to financial problem, I told her to make a promisory note in
order to get her module but until now she has no record of receive and return of module.

Name: Katrina Andrada

Address: Brgy Canmoros, Sitio Nabuswang
Reason: For her contact information, I already sent a request through her schoolmate near in their home
place and still I didn’t received any response yet. No attendance of parent from the last Parent’s
orientation (That is why I don’t have any contact with this student). She received her Module 1 based on
the record and no record of returned module 1 and release of Module 2 and 3.

Address: Brgy Canmoros Sitio Nabuswang
Reason: For his contact information, I already sent a request through his schoolmate near in their home
place and still I didn’t received any response yet. No attendance of parent during Parent’s orientation
(That is why I don’t have any contact with this student). No record of return of Module 1 and receive
module 2 and 3.

Reason: The student is currently working. He said that he will get his module as long as he has money to
pay for his modules and I told him to make a promisory note in order to get his module but until now he
hasn’t get his modules. I already message him through messenger and he left my message seen* and also I
already sent request through his classmate to give me his contact info from his family but until now I
didn’t get any response.
