Objective:
- By completing the exit ticket, the student will be able to demonstrate their understanding
of how our communities can change over time.

Rationale:
- The formative assessment I choose for this lesson was a google form. I choose this
because the students are familiar with the format and I believe that consistency is key for
my students. I find that this format is easy to give to students and it is in a way that the
students are able to understand easily. The quiz will give me results that will be my
starting point when planning my next lesson. The quiz is asking the students to list one
way that the community changed in their own community, and how it changed the
read-aloud ​The Little House ​by Virginia Lee Burton. The concept of identifying changes
ina community was practice many times during the lesson and was a great way to assess
the students’ overall understanding of the focus of that lesson.

Math

Objective:
- By participating in the Kahoot game, the students will be able to demonstrate their
understanding of addition with regrouping by using base ten blocks.

Rationale:
- The formative Assessment that I choose is a Kahoot review game. I decided to choose
this format because this is a very engaging way to assess the students effectively. The
students are asked a couple of questions that will check their overall understanding of
how to solve an addition problem while using regrouping. The question asks them to use
the images of the base ten blocks to solve the problem. Using Kahoot to assess math has
been very effective so far in the year, so I wanted to keep it consistent.

Science

Objective:
- By completing a google form, the students will be able to demonstrate an understanding
of what a life cycle is and the steps involved.

Rationale:
- The formative assessment I choose for this lesson was a google form. I choose this
because the students are familiar with the format and I believe that consistency is going
to be the best for my students overall. The student has been assessed in science through
google form in the past and it was very successful. Another reason why I thought a
google form would be the best way to go is because of the question type options. Not
only have I been able to assess them using a short answer question, but I also am able to
use checkboxes. I believe that this format was the best way to assess my students on
certain steps.
