The patient presents with dry lips, cracked mouth, red strawberry tongue, and a deep red oral mucosa with sores. The objective is to improve the impaired oral mucous membrane related to inflamed oral tissue. Short term goals include reporting less discomfort after 24 hours of nursing intervention and maintaining an intact oral mucosa free of ulceration after 2 days. Interventions include independent feeding of soft, nonirritating foods and cool liquids to decrease irritation and maintain hydration. Regular oral care with alcohol-free mouthwash and a soft-bristle brush is also planned to limit infection and irritation while the oral mucosa heals.
