Dear Ma’am/Sir:

I withdrew stocks of Drive Max Capsule last Friday, September 27, 2019 to deliver the
stocks to Farmania ZDM because they ordered 3 pads of capsule, but unexpectedly,
when I opened and checked the stocks, there were only 9 pads in the box.

I have also delivered 5 pads to LNA Pharmacy yesterday.

In this matter, I would like to inform you that the pad left in the box is only 1 instead of
2 pads.

I hope you understand that the incident happened due to company’s error and I’m not
liable to this.
