transportation and difficulties of the teachers. Efforts were exerted just to

teach the pupils in the said school. Barangay Domogok and the

Municipality Siayan but such were held futile due to the major

inaccessibility of farm to market road.

This certificate issued upon the request of the teachers in Basak

Elementary School for legal purposes.

travelling of teachers due the dangerous and extreme weather condition

like floods, landslide due to heavy rain falls. Efforts of the teachers continue

to support and provide quality education of the pupils in Basak lElementary

School.

Elementary School for legal purposes.

This refers to the incident happened on June 10,2015 at around 12:45 0’clock in the afternoon
when the building of Basak Elementary School was destroyed by landslide due to heavy rain
falls. Fortunately, nobody harmed of the said incident. Copies of the photographs of the School
building are hereto attached for your reference

Efforts were exerted just to save the building with the aid of the officials of Barangay Domogok
and the Municipality of Siayan but such were held futile due to the major damage of the building
caused by the said catastrophe which rendered it as totally unserviceable.

Previously ,the said School building was utilized for Grade 2,3 and 4.For the meantime ,pupils
who are used to occupy the said building are now currently conducting their classes in a public
market and church of Alliance in order to hamper their continuing their Education.

In view of the above aforementioned ,we humbly request from your good office for the major
repair of the said building or else demolition ,if possible immediate repair or replacement of one
(1)Unit School building with two(2)classrooms for the conducive learning and education of the
pupils of Basak Elementary School, Basak , Domogok, Siayan, Zamboanga del Norte.
