DEED OF REAL ESTATE MORTGAGE

KNOW ALL MEN BY THESE PRESENTS:

This contract of REAL ESTATE MORTGAGE, made and executed by and between:

____________________________, Filipino, of legal age, single/married to

____________________________________________________________ with residence
address at _____________________________________________________________
hereinafter called and referred to as MORTGAGOR; and BUREAU OF JAIL
MANAGEMENT AND PENOLOGY MULTI-PURPOSE COOPERATIVE, duly registered
before the Cooperative Development Authority with principal office address at 144
Mindanao Ave., Quezon City, hereinafter called and referred to as MORTGAGEE,

WITNESSETH:

That the Mortgagor does hereby convey by way of REAL ESTATE MORTGAGE
unto the Mortgagee the following described real property, situated in
_______________________, together with the improvements, to wit:

Of which real property the MORTGAGOR is the registered owner in accordance

with the provisions of the Land registration Act, His/Her title thereto being evidenced by
Transfer/ Original Certificate of Title No. _____________, of the Land registry of
__________________.

That this real estate mortgage is given as a security for the payment of the mortgage
of certain promissory note, dated ________________ for the sum of _____________ PESOS
(P ___________________), with interest thereon at the rate of ____ per centum (______%)
according to the terms thereof and in the words and figures as follows:
 “ FOR VALUE RECEIVED, I, _____________________________, of legal
age, Filipino and residing at _____________________________________, do
hereby promise to pay the BJMP Multi-Purpose Cooperative or order the sum of
_________________________ within _______________________ years, with
interest without the need of express demand, with the understanding that to enforce
the payment of this promissory note in case of failure to pay on the agreed terms, I
execute likewise a Deed of Real Estate Mortgage on said Real Estate Property
described above, with all the improvements therein, in favor of said BJMP Multi-
Purpose Cooperative.”

That the conditions of this real estate mortgage are such that if the mortgagor shall
well and truly pay or cause to be paid unto the mortgagee the aforesaid sum with accrued
interest, then this mortgage shall be of no further force’s effect; otherwise, the same shall
remain in full force’s effect’s shall be enforceable in the manner provided by law.

IN WITNESS WHEREOF, the mortgagor has hereunto set his hand, this _____ day
