MONITORING NOTICE

The Director General of Public Health, with whom the power is vested by Section 12(a) of
Public Health Protection Act (Law Number 7/2012), is taking the following measures in
coordination with the competent authorities, to investigate and monitor all individuals
currently residing in:
NO: 72, Dheyliyaage, DH. Kudahuvadhoo

Therefore, all individuals currently residing in the building/apartment is subject to

investigation, monitoring and treatment (if required) until further notice, and hereby
mandates the residents to adhere to the below mentioned obligations, for a 14 (fourteen) day
period or until further notice from the Health Protection Agency, from the date of issuance of
this Notice.

1. No one will exit or enter the building/apartment, except by approval of the Health
Protection Agency.
2. The residents of the building/apartment will cooperate with competent authorities to
facilitate and abide by all instructions.

The Director General of Public Health mandates the residents of the building/apartment to
observe strict adherence and give immediate effect to these measures, to prevent and contain
the possible transmission of COVID-19, till further notice by the Health Protection Agency.
