EQ-5D Limesurvey – Installation instructions

These instructions assume that you have a working LimeSurvey environment. Here you can find more details.
Minimum System Requirements: LimeSurvey Version 3.0

Introduction
Please follow the below steps in the described order to import EQ-5D for LimeSurvey.
You can import the EQ-5D instrument in your LimeSurvey application in 2 ways: (1) as a standalone EQ-5D survey, or
(2) incorporate it in one of your existing surveys.

Upload Theme
1. Configuration > Themes > Survey Themes > Import (see Figure 1 and 2)
2. Choose file: EQ-5D-LSTheme.zip
3. Select Import

Import EQ-5D – 2 possible ways

You can import the EQ-5D instrument in your LimeSurvey application in 2 ways: as a Survey or as a question Group.
OPTION 1: You should import it as a Survey when you want to create a standalone EQ-5D survey.
OPTION 2: You should import it as a Group when you want to incorporate EQ-5D in one of your existing surveys.

OPTION 1: Add EQ-5D as a standalone questionnaire – import as a Survey

1. Go to: Surveys > Import > Choose file (see Figure 3)
2. Select your LimeSurvey Survey (.lsa extension) file (e.g. EQ-5D-3L_survey.lsa)
3. Select Import survey

OPTION 2: Incorporate EQ-5D into an existing LimeSurvey questionnaire – import
as a Group
1. Go to: Surveys (Figure 4)
2. Select the Survey where you want to include EQ-5D in. Make sure this Survey is not active!

3. Select List question groups from the left menu and select import a group (Figure 5)

 4. Repeat the steps a and b below for all 6 files …01-MO.lsg to …06-VAS.lsg (See figure 6) in that exact order:
a. Select your LimeSurvey Group (.lsg extension) file
b. Select Import

End URL
When adding EQ-5D as a standalone survey, the End URL has been set to http://eq-5d.axentiva.com/ by default, this
is the EQ-5D Limesurvey demo page. feel free to modify this to any other value. You can do this here:
1. Surveys > [Select Survey you want to modify]
2. Select Survey text elements from the left menu and modify the End URL (Figure 6)
3. Select Save
