9. Self Appraisal(Key Job responsibilities Performed during the assessment Period)

1. Undertake HIV testing according to standard laboratory procedure.


2. Keep the facility neat and clean at all times.
3 .Ensure that adequate stock of consumables and rapid HIV diagnostic kits are available in
the ICTC.
4.Keep a record of HIV test results as well as a stock of rapid HIV diagnostic kits and
Consumables.
5. Ensure the maintenance of all laboratory equipment.
6. Scrupulously follow internal and external quality assurance procedures.
7.Follow universal safety precautions and strictly adhere to hospital waste management
Guidelines.
8. I have been available in the ICTC- lab as per the specified timings
9. Blood took from clients who had got per –test counseling about HIV
10. Clients ID number noted and labeled on sample container.
11. Run the test with worksheet as per NACO guidelines and standard operating
procedure.

16. External Quality assessment and panel testing are followed with SRL, trichy

18. Equipment maintenance and calibration are done properly.


19. HIV kits are kept in their optimal temperature.
20. All registers are updated regularly.
21. Needle stick injury is registered and post exposure prophylaxis given to affected
hospital workers.
22. Kit stocks are maintained and updated daily.
23. Internal quality control done for every new box kit and registered.
24. Occurrence management problems are rectifying with through proper channel.
25. IEC materials and universal work precautions posters are displayed in lab.
26.Every week – Tuesday , I have sent kit stock message DAPCU
27. Monthly lab performance report sent to SRL.
28. I have coordinated with counselor to ensure to given the best care to the clients from
health care facilities, ART centre, RNTCP.

10. What are the activities which you carried out which were outside the scope of your key
responsibilities:

 Sputum AFB test has been done by me to all cases referred by the medical officer. (Average
cases per month >150).
 HIV/AIDS awareness programmed conducted world AIDS day in our hospital.
 TB Awareness programmed conducted world TB day in our Hospital.
 Involve worked in VBD campaign many colleges.
 Hospital survey taken in corporation word.
