COMPLAINT-AFFIDAVIT
I, ANNA JUAREZ, of legal age, Filipino, married and a resident of San Antonio
Poblacion Calabanga, Camarines Sur, and after having sworn in accordance with law,
do hereby depose and state:
1. That I am accusing Juan Dela Cruz, a resident of San Pablo Calabanga,
Camarines Sur of Robbery.
2. That on Dacember 10, 2020 at about 10:00 o’clock in the evening, we are
already sleeping with my husband in our bed;
3. I was awaken by the noise in our kitchen, because I was scared to go downstairs
I just ignore it thinking that it was just a cat or mouse. Early in the morning when I
go to the kitchen to drink and prepare for breakfast, I noticed that our window
was broken.
4. So I asked my husband to check the CCTV footage, then we saw a familiar face
of a man smash our window and went inside our house. He roam around like his
looking for something to take, then he took the laptop of my daughter that was
left at the table and her bag that was placed on her room.
5. That morning our neighbor Rianne told us that he saw Juan smashing our
window and went inside our house, then we confirm it based on the CCTV
footage. So we call the police and report the incident.
6. That I am executing this Complaint-Affidavit for the purpose of filling the
complaint for Robbery against JUAN DELA CRUZ.

COMPLAINT-AFFIDAVIT
I, JUAN MERCEDES, of legal age, Filipino, married and a resident of Sta. Cruz
Poblacion Calabanga, Camarines Sur, and after having sworn in accordance with law,
do hereby depose and state:
1. That I am accusing Antonio Juan , a resident of San Francisco Calabanga,
Camarines Sur of Arson.
2. That on February 3, 2020 at about 12:00 o’clock midnight, we are already
sleeping with my husband in our bed;
3. My wife woke me up because she smells a smoke coming from our house. We
hurriedly check where the smoke came from then we saw our kitchen burning.
4. We woke our kids and went outside the house for our safety and we call for help
to our neighbor, some of them help us and call the Bureau of Fire and protection.
Thankfully the fire was quickly knocked down and only some parts of our house
was damage.
5. That after the fire was knocked down our neighbor Renato approach us and said
that he saw Antonio roaming around our house later that afternoon before the
incident happened.
6. It was confirmed when we watched the CCTV footage of our house and I was
sure that the accuse is really the suspect because we have a family feud
regarding the land that we owned.
7. That I am executing this Complaint-Affidavit for the purpose of filling the
complaint for Arson against ANTONIO JUAN.
