Before staring the practical assessment you have to do inspection of any workplace

as per your convenient, it might be your workplace, your friends or any other. The
main aim of this inspection is to identify the unsafe acts;unsafe conditionsat the
workplace that could lead to accident, also note the good practices.

Section 1: Observation
Factors to be considered before writing Observation sheet:

 Write 25 to 27 Hazard identified in Inspection, your hazard should include


unsafe acts and unsafe conditions only, no preventive measure should be
considered here.
 Write at least 2 to 3 good practices in between hazards in a random way.
 Your total number of Observation should be 27 to 30 only, not more than 30
 Your hazards should reflect all categories like chemical, physical, biological,
ergonomical and psychological hazards; this is to reflect your knowledge of
all the categories of hazards.
 Your series of observation should be in systematic way covering all area or
department one by one, as this report is based on workplace inspection so
your series of observation should reflect real inspection.( starting from
entrance and covering all area and activity in a series)
 Do not cover more than 2 hazards in a single machine or activity, so that you
can cover all areas of concern.
 While writing the Hazard and consequences your sentence should reflect,
what is the hazard, where is it, what will be the consequences
 You have to write atleast 2 controlsfor hazards that are immediate control
and long term control, better write 3 controls that is immediate control, short
term control and long term control.

Section 2: A) INTRODUCTION B) EXECUTIVE SUMMARY


A) Introduction

In this section you should write when the inspection was carried out, location,
the site famous for, what are the different activities carried out, different
equipment used and the inspected area.
B) Executive Summary

This section has to be filled at the last after finishing the recommendation
section. You have to highlight minimum 5 area of concern on which
management has to focus. Each 5 area of concern should be further described in
paragraph that what management has to do for improvement this means you
have to right only recommendation regarding that particular issue

(Please note you don’t have to describe hazards in paragraph, only describe the
recommendation in technical terms)

Section 3: Main Finding of the Inspection


First student have to extract atleast 5 critical hazards from the 30 Observation
written in section 1

You have to describe the entire five critical Hazards as mentioned below

1) In 1st paragraph describe the hazard in technical term in 3 lines


In 2nd paragraph write down the particular breach of law with article number
and whole sentence of the article number
In 3rd paragraph write the recommendation including approximate cost and
benefits
So in this way your 1 critical hazard will have 3 paragraphs
2) Write 3 paragraph as mentioned
3) Write 3 paragraph as mentioned
4) Write 3 paragraph as mentioned
5) Write 3 paragraph as mentioned

Section 4: A) Conclusion B) Recommendation


A) Conclusion

In this section you have to convince the management for allocation of fund, so
should start with some good things of the organisation and appreciate them, then
highlight the 5 issues in the organisation, you can use moral legal and
financialreasons to convince the management, you can show the benefit of
following the recommendation in this report, u can use direct and indirect losses
that can occur after the accident.

Note: This section should be only written on 1 page

In this section you have the fill all the tables like

Approximate cost
Priority (High)
Target date(Should consider the time given in observation and give a
particular date +5 to 10 days)

Note: The series of Hazard written in main finding should reflect the same
series in executive summary and recommendation section.
