 CSC form 212 (revised 2005) in two copies with latest 2x2 I.D. picture.

 Certified photocopy of PRC certificate of registration of license.

 Certified photocopy of rating obtained in licensure

 Examination for teachers/professional board.

 Certified true copy of transcript of record baccalaureate

 Certificate of authentication and verification

 Copies of service record, performance ratings and school clearance for those with

teaching experience.

 Certificate of specialized trainings, if any.

 Certified copy of voter’s ID.

 N.B.I. clearance

 Omnibus certification of authenticity and veracity of all documents submitted.
