AFFIDAVIT
(One and the Same Person)

I, ANGELICA ISABELINA MONTERO DELOS SANTOS, Filipino, of legal age,

married, and with residence and postal address at Block 40 Lot 15 Camella Heights Subdivision,
Cararayan, Naga City, after having been sworn to in accordance with law, do hereby depose and
state:
That my maiden name is Angelica Isabelina Floranza Montero, attached herein is a
photostatic copy of my Tax Identification Card marked as Annex “A” and forming part hereof;
That on 29 January 2020, I was married to Mr. Ian Javier Delos Santos, in accordance
with the Evangelical Christian rites held at United Church of Christ Philippines, Peñafrancia
Ave., Naga City;
That after such marriage rite, I acquired and now use my husband’s family name “Delos
Santos”
That as a consequence thereof, I started using my husband’s family name in my
identification cards as well as transactions I enter into and at present known to be Angelica
Isabelina Montero Delos Santos, attached herein is a photostatic copy of my Philippine Passport
bearing the surname “MONTERO” and marked as Annex “B” and forming part hereof;
That the names Angelica Isabelina Floranza Montero and Angelica Isabelina Montero
Delos Santos are one and the same person;
That I am executing this Affidavit of One and the Same Person to attest to the truth of the
foregoing statements and for whatever legal purpose this may serve.
Further affiant sayeth naught.
