TO SUBMIT ENROLLMENT REQUIREMENTS

TO: THE SAN BEDA UNIVERSITY ADMISSIONS CENTER

undertake as follows:

1. I am informed that enrollment in San Beda University for AY 2020-2021 is being done online in view of
the present COVID-19 crisis;

2. In view of above, I understand that I am allowed to enroll CONDITIONALLY as a first year student of San
Beda University-College of Law this Academic Year 2020- 2021, even without physically submitting the
following, which are indispensable requirements for admission:

____ Original Transcript of Records (TOR) with the remarks “Copy for Evaluation Only”
____ Certificate of Graduation (only if TOR is not available)
____ Certificate of Eligibility to Transfer/Transfer Credential
____ Original and photocopy of PSA Birth Certificate

3. In the meantime, I am required to submit a scanned copy of each of the enrollment requirements
stated in item number 2 of this undertaking;

4. I undertake to submit the above-mentioned records on or before July 31, 2020;

5. In the event that any of the requirements is found to be fake, tampered or suffers from any infirmity
that disqualifies me from pursuing Legal education, my enrollment shall be deemed voluntarily withdrawn
and I shall be dropped from the list of students of San Beda University, with forfeiture of payments made
pursuant to school policy.
